    Description / Table of Contents: Summary The purpose of our studies was to investigate experimentally the dose-dependent effects of extracorporeal shock waves on tendon and bone and to unveil therapeutic possibilities in tendopathies and pseudarthroses. In animal experiments, both positive and negative influences were exerted by shock waves, depending on the initial situation and on the power of the applied shock waves. In prospective clinical trials positive effects were found in the treatment of persistent tennis elbow, plantar fasciitis, calcifying tendinitis, and pseudarthrosis. Our data show that extracorporeal shock waves may provide analgesic, resorptive and osseoinductive reactions with nearly no side effects. However, the high cost of apparatus and staff prevents a routine application. Extracorporeal shock waves thus remain a last alternative before the indication is made for an operative procedure.
    Notes: Zusammenfassung Ziel unserer Untersuchung war es, dosisabhängige Effekte der extrakorporalen Stoßwellentherapie (ESWT) bei der Behandlung von Sehnen und Knochen aufzuzeigen, Schädigungsmöglichkeiten darzustellen und so Einsatzmöglichkeiten sowohl bei den im orthopädischen Alltag häufigen Tendopathien als auch bei der Pseudarthrosenbehandlung zu beleuchten. Tierexperimentell konnten an Sehnen und an Knochen, in Abhängigkeit von der Ausgangssituation und der Stoßwellenstärke, sowohl positive als auch negative Effekte beobachtet werden. In prospektiven klinischen Studien wurden die Anwendungsmöglichkeiten extrakorporaler Stoßwellen unterschiedlicher Energiedichten untersucht. Positive Effekte wurden bei Patienten mit Tennisellenbogen, plantarem Fersenschmerz, Tendinosis calcarea der Schulter und bei Pseudarthrosen erzielt. Die dargestellten Ergebnisse zeigen, daß mit der extrakorporalen Stoßwellentherapie ein nebenwirkungsarmes Verfahren existiert, um einerseits analgesierend einzugreifen und um andererseits resorptive bzw. osteogenetische Prozesse einzuleiten oder zu beschleunigen. Der noch immer hohe apparative und personelle Aufwand steht einer Routineanwendung entgegen. So ist die ESWT in der Orthopädie derzeit weniger eine weitere konservative Behandlungsform, sondern vielmehr letzte Alternative vor einem operativen Eingriff.

Le but de cette étude est d’évaluer les effects des ondes de choc de basse énergie sur les douleurs persistantes de l’épiconcyle afin de préciser leur utilisation en orthopédie. On a divisé 50 patients en deux groupes selon le nombre des ondes de choc appliquées. Le groupe n° I qui a re*u trois fois 100 impulsions à 0.08 mJ/mm 2 montrait significativement des meilleurs résultats après trois et douze semaines que le groupe n° II qui a re*u trois fois 10 impulsions à 0.08 mJ/mm 2 . Globalement nous avons trouvés des résultats bons et excellents chez 56% des patients du groupe n° I comparé avec 20% chez des patients du groupe n° II.
    Notes: Summary. Fifty patients who suffered from persistent tennis elbow for more than 12 months, and were referred for surgical treatment, were assigned at random to 2 groups of low-energy extracorporal shock wave therapy. Group I received a total of 3000 impulses of 0.08 mJ/mm 2 ; group II (controls) 30 impulses of 0.08 mJ/mm 2 . Follow up was after 3 and 12 weeks. We found no significant differences between the 2 groups before treatment, there was but significant relief of pain and improvement of function in group I with good or excellent outcome in 56% at the last evaluation.

However, mutation-selection balance generates a certain amount of additive-genetic variance, so that even parent-offspring measures of heritability may be greater than zero at equilibrium. Intra-generation heritability may also be non-zero, providing the potentials for genetic change following environmental change.

    Notes: Summary It has been claimed that Borage (Borago officInalis L.) has a multifactorial self-incompatibility system. Such systems may have a high level of ineffective pollination, and we show that this is the case in borage. The ranking of seed set from highest to lowest is as follows: bee-pollination; natural pollination in the absence of bees; artificial cross-pollination between unrelated plants; artificial cross-pollination between related plants; artificial self-pollination. In diallel crosses, significant parental effects were detected but no consistent patterns of seed set, which suggest a simple self-incompatibility system, were detected. The level of outcrossing with natural pollination was very variable but greater than 50%. Thus, there appears to be no straightforward self-incompatibility system in borage.

    Description / Table of Contents: Zusammenfassung Bei korrekter Durchführung einer antegraden Sklerosierung nach Tauber bei einem 38 jährigen Patienten mit einer Varikozele Grad 3 kam es zu einer hämorrhagischen Infarzierung des Hodens durch den kompletten Verschluß des venösen Rückstroms. Aufgrund dieser aufgetretenen Komplikation, die bisher nicht beschrieben wurde, mußte eine Semikastration erfolgen. Bei der Aufklärung zur antegraden Sklerotherapie sollte diese Komplikation beachtet werden.
    Notes: Summary A 38-year-old patient with a grade 3 testicular varicocele was treated by antegrade scrotal sclerotherapy. Although performed technically accurate, antegrade sclerotherapy led to a haemorrhagic infarction of the testis by complete occlusion of venous drainage. The testis had to be removed. This complication has not been described yet and patients should be informed about this rare event.

    Description / Table of Contents: Abstract The following report concerns a 52-year-old patient suffering from coronary disease, who underwent coronary surgery in a department of heart- and vascular surgery. The implantation of a femoropopliteal bypass with anastomosis in the first popliteal segment was done during the same operation due to a long-distance occlusion of the superficial femoral artery on the right side. Some months later the patient consulted us, complaining of severe pain in his right leg and an immensly reduced walking distance. The arteriography showed an arteriovenous fistula of the right leg after implantation of a femoropopliteal bypass. The cause for this was an anastomosis of the distal part of the bypass with the popliteal vein. The patient underwent surgery again with ligature of the distal part of the bypass and reanastomosis with the original popliteal artery in the first segment. The arteriography done during the operation then showed correct conditions. Afterwards the distance the patient could walk was prolonged and he was without pain.
    Notes: Zusammenfassung Wir berichten im folgenden über einen 52jährigen Patienten, der sich bei einer bestehenden koronaren Herzkrankheit in einer auswärtigen Klinik für Herz- und Gefäßchirurgie einer 4fach-ACVB-Operation unterzogen hat. Bei gleichzeitig bestehender peripherer arterieller Verschlußkrankheit rechts führend (Stadium IIb–III) mit langstreckigem Verschluß der A. femoralis superficialis erfolgte in gleicher Sitzung die Implantation eines femoropoplitealen Bypasses mit supraglenuidalem Anschluß rechts. 4 Monate postoperativ stellte sich der Patient in unserer Klinik vor und berichtete von einer deutlich abgenommenen Gehstrecke und Schmerzen im rechten Bein. Die von uns durchgeführte DSA zeigte die Ausbildung einer postoperativen arteriovenösen (A.V.-)Fistel nach femoropoplitealen Bypass. Ursache hierfür war die distale Anastomisierung des angelegten Bypasses mit der V. poplitea. Es erfolgte in unserem Hause eine Dissektionsligatur im Bereich des Bypasses distal und eine Reanastomosierung der Gefäß-prothese auf die A. poplitea im 1. Segment, die intraoperative Angiographie zeigte jetzt korrekte Verhältnisse.

    Notes: Summary In 50 female Sprague-Dawley rats the absorption of Cisplatinum through the bladder wall was studied. Under different conditions, including cystitis and electrocoagulation of the bladder mucosa, the absorption was low and the measureable serum concentrations did not exceed 2.64 μg/ml. The influence of Tween 80 — a non-ionic surfactant — on the absorption is investigated. The results are compared with those found for Adriamycin and Mitomycin C under identical conditions.

    Notes: Summary A comparative study was undertaken to investigate the application of a specially adapted microsurgical Neodymium Yag Laser system with a wavelength of 1,319 μm and a CO2 laser system for laser assisted microvascular end-to-end anastomosis (LAMA) of the rat femoral artery. Conventionally sutured anastomoses served as controls. Postoperative investigations included patency tests, light microscopy and tensile strength measurements. Both laser systems seem to be equally suitable for LAMA: The patency rates do not differ from those of sutured anstomoses and formation of microscopically small aneurysms occurred predominantly in control animals and only once in laser groups. The clamp time needed for LAMA was half the time that was needed for sutured anastomoses. Wound healing in all groups was similar with less fibrotic reactions and less foreign body granulomas in laser groups. At all intervals tensile strength was significantly higher for sutured anastomoses while differences between the CO2- and the ND: Yag-laser groups were not statistically significant. Potential applications in urology include microvascular anastomoses in erectile dysfunction, pediatric and reconstructive urology.

    Notes: Summary A comparative study was undertaken in 81 rats to investigate a CO2 and Nd:YAG laser system for laserwelded anastomosis of the femoral vein. Conventionally sutured anastomoses (CMSA) served as controls. Laserwelded anastomosis (LAMA) was easier and could be performed 30% faster than CMSA. Postoperative investigations included patency tests, postmortem examinations and light and electron microscopy. Aneurysms were not seen. Foreign-body reaction was more pronounced in CMSA. Patency rates for CO2-LAMA and CMSA were equal, whereas Nd:YAG-LAMA resulted in significantly higher rate of early postoperative thrombosis (P〈0.01). Because of its physical properties, the CO2-laser system seems to be better suited for laser welding of delicate structures such as the rat femoral vein.

    Notes: Abstract The aim of this prospective single-blind pilot study was to explore the pain-alleviating effect of low-energy extracorporeal shock wave therapy (ESWT) in painful heel associated with inferior calcaneal spurs. Thirty patients who suffered from persistent symptoms for more than 12 months qualified for low-energy ESWT and were assigned at random to two groups, real or simulated ESWT. Before beginning the treatment, any other therapy was stopped for a period of 6 weeks. The shock waves were applied by an experimental device allowing exact localization through an integrated fluoroscopy unit. Patients were treated three times at weekly intervals. Each time 1000 impulses of 0.06 mJ/mm2 were given around the heel spur. Follow-ups were done after 3, 6, 12 and 24 weeks. Patients of the placebo group who did not improve at the 6-week follow-up were then offered ESWT therapy and were checked at 3, 6, 12 and 24 weeks after the last treatment. Whereas we noticed no significant differences between the groups before ESWT, there was a significant alleviation of pain and improvement of function at all follow-ups in the treatment group.
