    Description / Table of Contents: Abstract Asymptomatic microhematuria is a common reason for a urological consultation. Uncertainty prevails as to how meticulous the work-up must be, to not miss relevant or even life-threatening underlying diseases. To date, the Urological Associations have not released any guidelines to which extent patients need to be examined for asymptomatic microhematuria, which therefore is managed individually by each urologist. There are various potential examinations that can be applied, ranging from a clinical examination to a kidney biopsy. After reviewing the litheratur, an algorithm has been developed, which should assure diagnosis of serious disease and at the same time avoid costly, unpleasant and unnecessary examinations.
    Notes: Zusammenfassung Die Abklärung einer asymptomatischen Mikrohämaturie ist ein häufiger Zuweisungsgrund an den Urologen. Unsicherheit herrscht dabei über das Maß der erforderlichen Abklärungen, immer beherrscht von der Sorge relevante oder sogar lebensbedrohliche Erkrankungen zu übersehen. Derzeit stehen noch keine Guidelines der urologischen Fachgesellschaften zur Verfügung, sodass der Abklärungsmodus der asymptomatischen Mikrohämaturie von den Untersuchern individuell gehandhabt wird. Die Bandbreite der Abklärungen reicht dabei von der klinischen Untersuchung bis zur Nierenbiopsie. Anhand einer Literaturübersicht wird ein Abklärungsalgorithmus vorgestellt, welcher dem Bedürfnis nach Ausschluss einer schwerwiegenden Erkrankung nachkommt, und gleichzeitig helfen soll eine für Patient und Gesundheitsträger belastende teure Überdiagnostik zu vermeiden.

    Notes: Abstract  Limited data exist on therapeutic options for established hepatic veno-occlusive disease (VOD) in pediatric patients after stem cell transplantation (SCT). In this report, we present data on the successful treatment of VOD in three children following allogeneic SCT and report the duplex ultrasound criteria for the confirmation of the diagnosis and for the evaluation of the treatment progress. All patients were 〈2 years at the time of transplantation and had received preparative regimens containing busulfan and cyclophosphamide. There were no known pretransplant risk factors for VOD. Allogeneic stem cell transplantation was performed from a sibling donor for CMML and from unrelated donors for Wiskott-Aldrich syndrome and familial hemophagocytic lymphohistiocytosis (FHL). The onset of first clinical symptoms of VOD (as defined by the Seattle and Baltimore criteria) was relatively late in all three patients (days +19, + 20, and +25, respectively). Time from onset of first symptoms until confirmation of diagnosis by serial duplex ultrasound examination was 4–11 days. Duplex ultrasound criteria are as follows: complete change of direction of blood flow in the portal vein, decrease of flow in the hepatic veins, and development of collateral circulation. Treatment was initiated upon confirmation of VOD by continuous infusion of prostaglandin E1 (initial dose 0.075 μg/kg/h) in addition to low-dose heparin (100 units/kg/d). Treatment was continued at the maximum tolerated dose of 0.3–0.5 μg/kg/h of PGE1. After 9, 14, and 25 days of treatment respectively, normal portal vein flow was restored and treatment could be discontinued. All three patients are alive and well without apparent sequelae.

    Description / Table of Contents: Zusammenfassung Es wurde die Eignung der hitzestabilen alkalischen Placentaphosphatase (HSAP) als Gradmesser der Placentafunktion bei 350 Patientinnen anhand von insgesamt 2029 Seren geprüft. In Normalfällen zeigte sich ein kontinuierlicher Anstieg der HSAP mit nur geringen Abweichungen vom einmal eingeschlagenen Trend. Pathologische Kurvenbilder mit Werten außerhalb des Normbereichs oder Trendänderung im Sinne eines Anstiegs oder Abfalls fanden sich vor allem bei Spätgestosen und Hypertonien, und zwar in 85% der Fälle, in denen eine gleichzeitige Placentarinsuffizienz zu einer Mangelentwicklung oder zum intrauterinen Fruchttod führte, und in knapp 30% der Fälle, bei denen trotz einer Gestose eine allfällige placentare Dysfunktion klinisch nicht faßbar wurde. In Normalfällen war die HSAP in 20% pathologisch verändert. Die prognostische Aussagekraft in bezug auf den fetalen Zustand im Rahmen eines EPH-Syndroms schien zwischen der 26. und 36. Schwangerschaftswoche größer zu sein als gegen den Termin zu. Der einfachen Bestimmung wegen eignet sich die HSAP vor allem als Screening-Methode in normalen und pathologischen Schwangerschaften. Eine abnorme Verlaufskurve als Hinweis auf eine möglicherweise gefährdete Gravidität sollte in erster Linie zu einer Erweiterung des Spektrums der Überwachungsmethoden führen und nicht sofortige therapeutische Konsequenzen zur Folge haben.
    Notes: Summary The predictive efficacity of the heat-stable alkaline phosphatase (HSAP) for monitoring the placental function was studied in 2029 serum samples from 350 women. In normal pregnancies there was usually a continuous increase of the HSAP activity. In 85% of the pregnancies complicated by hypertension or EPH-gestosis with significant placental dysfunction, values above or below the normal range or marked changes of the slowly rising trend in serial estimations could be found. The same observations, however, were made in about 30% of these cases without apparent insufficiency of the placenta and in 20% of the normal cases. Generally the predictive value seemed to be better between the 26th and 36th week than near term. It was concluded that the estimation of the HSAP activity may be used as a very simple screening method in normal and pathological pregnancies. Abnormal results, however, should rather signal a strict monitoring and not therapeutic decisions.
