    Abstract: This paper describes an automatic parameter optimization method for anisotropic diffusion filters used to de-noise 2D and 3D MR images. The filtering process is integrated into a closed-loop system where image improvement is monitored indirectly by comparing the characteristics of the suppressed noise with those of the assumed noise model at the optimal point. In order to verify the performance of this approach, experimental results obtained with this method are presented together with the results obtained by median and k-nearest neighbor filters.

    Notes: Summary A marked histochemical compartmentalization is visible in the substantia nigra of the squirrel monkey in sections stained for acetylcholinesterase (AChE). In nigral regions containing tyrosine hydroxylase-positive neurons, there are AChE-poor and AChE-rich zones, and many of the AChE-poor zones have the form of narrow fingers extending ventrally into an AChE-rich matrix (Jimenez-Castellanos and Graybiel 1987b). The study reported here was carried out to determine whether this histochemical heterogeneity of the primate's substantia nigra is related to the known differentiation within its pars compacta of subdivisions projecting respectively to the caudate nucleus and to the putamen. Retrograde and anterograde labeling in the substantia nigra was elicited by tracer injections placed in the caudate nucleus or putamen and was plotted in relation to patterns of AChE staining and tyrosine hydroxylase immunostaining. Much of the labeling observed was organized according to borders visible with AChE histochemistry: labeled nigral neurons (and afferent fibers) tended to be clustered precisely within the AChE-poor ventrally-extending fingers or to be situated outside these zones. However, projection neurons in these ventrally-extending fingers were not exclusively related either to the caudate nucleus or to the putamen. After injections in the caudate nucleus, labeled neurons were predominantly in the AChE-poor fingers in some cases, but predominantly in AChE-rich nigral zones outside them in other cases. Labeling in and out of the ventrally-extending fingers, and along the edges of the fingers, also occurred following different tracer injections in the putamen. These findings confirm the independent clustering of nigrostriatal neurons projecting respectively to the caudate nucleus and to the putamen. The plan of nigrostriatal connections additionally appears concordant with the histochemical compartmentalization of the substantia nigra that can be detected with acetylthiocholinesterase histochemistry.

Le résultat de 78 arthroplasties de résection (technique de Girdlestone) réalisées sur 78 patients présentant un sepsis au niveau de leur prothèse de la hanche – moyenne du suivi 5 ans- a été analysé. Lors de l’arthroplastie de résection, les germes trouvés furent Gram positif dans 53% des cas, Gram négatif dans 33% et dans 12% des cas, ils présentaient une infection mixte. L’intervention de Girdlestone a permis de maîtriser l’infection dans 86% des cas et dans 83%, une maîtrise satisfaisante de la douleur. Mais au détriment du raccourcissement du membre (4.1 cm en moyenne) et obligeant le recours à un soutien externe pour marcher dans 100% des cas. Les auteurs ne trouvèrent aucune corrélation entre le type de germe et la permanence de l’infection ni entre le niveau de résection et le raccourcissement et ce dernier et le résultat fonctionnel. En conclusion, l’arthroplastie de Girdlestone est une méthode acceptable pour la maîtrise de l’infection et de la douleur.
    Notes: Summary. The results of 78 resections of the head and neck of the femur (Girdlestone pseudarthrosis) in patients with infected hip replacements were studied. The mean follow-up was 5 years. At the time of the resection, gram-positive organisms were found in 53% of the cases, gram-negative in 33%, and in 12% there were mixed flora. The Girdlestone pseudarthrosis controlled the infection in 86% and achieved satisfactory relief of pain in 83%. The mean shortening of the limb was 4.1 cm and every patient needed some type of external walking aid. We found no correlation between the type of organisms and the persistence of infection, nor between shortening and the functional results. The Girdlestone pseudarthrosis is an acceptable method of controlling infection and relieving pain after infection of a total hip replacement.

    Description / Table of Contents: Résumé Nous discutons trois aspects importants du traitement chirurgical de la pancréatite aigüe: le moment de l'opération, la technique à utiliser (en fonction des découvertes opératoires), les résultats. Après le traitement médical initial, le moment de l'opération doit être choisi en fonction des données cliniques et biologiques. La classification des pancréatites aigües en trois types de gravité croissante peut faciliter la décision. Si l'on trouve une pancréatite oedémateuse, irrigation et drainage suffisent. En cas de nécros distale, il faut faire une pancréatectomie gauche ou subtotale. Les lésions nécrotiques de l'isthme demandent une excision locale, éventuellement complétée par une pancréatectomie gauche. Les résections de la tête pancréatique ont été abandonnées à cause de leur mortalit é élevée. L'opération doit, à l'heure actuelle, se limiter à l'exérèse des tissus nécrosés (nécrosec tomie), avec drainage et irrigation. Il faut, de plus, faire dans tous les cas un drainage de la voie biliaire par cholédocotomie ou au moins cholécystostomie, un drainage de la zone nécrosée et une aspiration gastrique. Nous avons opéré en suivant ces principes 49 malades avec une mortalité de 36.7%.
    Notes: Abstract Three important aspects of the surgical therapy of acute pancreatitis are discussed: the time at which operation appears to be indicated, the procedure to be chosen (in accord with operative findings), and the results. After initial medical treatment, the timing of intervention should be based on clinical and biological information. Classification of acute pancreatitis in 3 degrees of severity may facilitate the decision. When edematous pancreatitis is found, irrigation and drainage are indicated. For distal necrosis, a left or subtotal pancreatectomy should be carried out. Necrotic lesions of the isthmus should be excised locally; if necessary, local isthmus excisions may be combined with left pancreatic resection. Resections of the head of the pancreas have been abandoned because of their high mortality rate; intervention is now limited to local excision of necrotic tissue (necrosectomy), with drainage and irrigation. In all cases, complementary biliary drainage by choledochostomy or at least cholecystostomy, drainage of areas of necrosis, and gastric suction are also mandatory. Forty-nine patients who were operated upon according to these principles with a mortality rate of 36.7 % are discussed.

    Notes: Abstract We present a case of intra-articular osteoid osteoma of the humerus simulating chronic monoarthritis of the elbow in an 18-year-old male right-handed recreational tennis player. CT revealed a well-defined nidus in the coronoid fossa. Microscopic examination of the synovium showed a lymphofollicular synovitis that resembled the synovitis in rheumatoid arthritis.

    Notes: Summary The effect of different densities of animals per cage on basal testosterone secretion and its response to some stimuli was studied in prepuberal male mice. Mice housed with 110 or 55 cm2 of floor space per animal showed the same basal serum testosterone and hCG-induced testosterone release. Likewise, in a second experiment, the same basal serum testosterone and a similar response to acute noise stress were found in mice housed at 132, 66 or 22 cm2 of floor space per animal. These results suggest that post-weaning crowding did not affect Leydig cell function in male mice.

    Description / Table of Contents: Résumé Dans cette étude nous avons étudié l'anatomie macroscopique (mésoscopique) de l'a. ophtalmique chez l'homme. Grâce au microscope et aux outils micro-chirurgicaux, nous avons pu obtenir des données sur l'origine et le trajet de cette artère et de ses principales branches collatérales. Nous avons comparé nos rśultats aux données précédemment rapportées et avons conclu que certains points pouvaient être mis en relief, malgré la diversité bien connue des variations anatomiques du système vasculaire. Ainsi l'a. ophtalmique apparaît habituellement comme une branche collatérale de l'a. carotide interne, bien que d'autres origines de cette artère aient été retrouvées. L'a. ophtalmique, dans son trajet intra-orbitaire, suit un trajet situé au dessus du n. optique dans la plupart des cas. Toutes les branches collatérales de l'a. ophtalmique, à l'exception des branches musculaires, présentent une grande constance.
    Notes: Summary In the present paper we have studied the gross (mesoscopic) anatomy of the ophthalmic a. in humans, using magnification by microsurgical systems to obtain data on the origin and course of this artery and its main collateral branches. Comparison of our results with previous reports indicates that, although the anatomical variations of the vascular system are well known, some patterns of frequency may be emphasised. Thus, the ophthalmic a. was usually found as a collateral branch of the internal carotid a., although other origins were also found. The ophthalmic a., once inside the orbit, followed a course above the optic nerve in most cases. All the collateral branches of the ophthalmic a., with the exception of the muscular branches, showed great constancy.

    Notes: Volatiles were removed from fish sauce by continuous-flow extraction with microbubbles of supercritical carbon dioxide. The extraction was done at 35 °C and CO2/sample flow ratio of 0.14 and 0.29 under pressures of 10 to 30 MPa. After the treatment at a CO2/sample flow ratio, 0.29 at 10 MPa, remaining percentage ((the concentration in treated sample/that in untreated one) × 100) was 5.2% trimethylamine, 8.0% S-methyl ethanethioate, 30% dimethyldisulfide, 55 to 61% aliphatic aldehydes, and 25 to 42% carboxylic acids. The increase in CO2 flow rate improved the extraction efficiency significantly, but no effect of pressure was observed. The odor intensities of treated samples were between 1/4 and 1/8 of the untreated fish sauce.
