    Notes: Summary A total of 28 clones were established from the PLC/PRF/5 hepatoma cell line by a plating procedure. All clones were found to secrete HBsAg into the supernatant culture fluids. Of these, one clone (No. 23) free of detectable mycolasma contamination and showing smooth epithelial morphology was selected for further study. Maximum accumulation of HBsAg occurred 9 days after sub-culture and intracellular antigen was detected by indirect immunofluorescence both in the cytoplasm and at the plasma membrane. Granules and perinuclear staining reactions were also observed in clone 23 cells and these findings are compared to the previously published properties of the parental PLC/PRF/5 cell line.

    Description / Table of Contents: Rèsum/`e Les observations hospitalières de 60 étudiants ouest-africains admis entre 1955 et 1965 dans un hôpital d'enseignement post-universitaire ont été étudiées. Une enquête longitudinale par correspondance a été entreprise et les observations se rapportant aux admissions ultérieures examinées. — Les problèmes diagnostiques ont été passés en revue et la signification des idées délirantes de persécution évaluée. Il a été constaté que celles-ci existaient chez presque tous les malades, quel que soit le diagnostic, mais qu'elles avaient une signification diagnostique en faveur de la schizophrénie seulement quand leur contenu était de type occidental, soit parce que les personnes accusées n'étaient pas africaines, soit parce que des appareils électroniques, etc., étaient mis en cause. — Il a été souligné que les idées d'influence ne devaient pas être confondues avec les idées indigènes d'empoisonnement et de magie. La survenue fréquente d'idées délirantes de grandeur chez les ouest-africains a été confirmée. — Cinq étudiants seulement réussirent leurs examens. Le diagnostic de schizophrénie et plus d'une seule admission à l'hôpital ont été trouvés liés à un échec pratiquement certain dans les études. — Les tests de Mill Hill et des Progressive Matrices se sont révélés très défectueux pour l'estimation du quotient intellectuel de ces étudiants. — Les problèmes de rapatriement ont été rapidement discutés. D'autres travaux suggèrent que la maladie actuelle et dans une moindre proportion la fréquence des rechutes peuvent être améliorées à la suite du rapatriement. Cependant cette éventualité doit être conseillée en prenant soigneusement en considération l'épreuve qu'elle représente pour le malade et ses chances de passer ses examens. Dans l'état actuel des choses, il semble raisonnable de recommander le rapatriement des schizophrènes pour lesquels un séjour de plus de 3 ou 4 mois à l'hôpital est nécessaire et des malades, quel que soit le diagnostic, qui ont été admis plus d'une fois à l'hôpital, à partir du moment où il apparaît peu probable qu'ils poursuivent leurs études avec succès.
    Abstract: Zusammenfassung Die Krankengeschichten von 60 west-afrikanischen Studenten, die zwischen 1955 und 1965 in eine Ausbildungsklinik für postgraduierte Studenten aufgenommen worden waren, wurden untersucht. Später wurden eine briefliche Nachuntersuchung durchgeführt und die Berichte von Wiederaufnahmen eingehend geprüft. — Es wurden diagnostische Probleme behandelt und die Bedeutsamkeit von wahnhaften Verfolgungsideen abgeschätzt. Letztere kamen unabhängig von der Diagnose bei nahezu allen Patienten vor. Es wurde ihnen nur dann eine diagnostische Relevanz für Schizophrenie zugemessen, wenn ihr Inhalt aus westlich-abendländischem Denken entstammte, dann nämlich, wenn die im Wahn beschuldigten Personen Nichtafrikaner waren oder wenn elektronische Apparate darin eine Rolle spielten. — Es wurde betont, daß Gefühle der Passivität nicht mit Vergiftungswahn und magischem Denken verwechselt werden dürfen. Es wurde bestätigt, daß Größenideen bei Westafrikanern normalerweise vorkommen. — Nur 5 der Studenten erwiesen sich im Examen als tauglich. Mit der Diagnose Schizophrenie und mit mehr als einer Klinikaufnahme ging zumindest ein gewisses Versagen im Studium einher. — Bei diesen Studenten erwiesen sich für die Messung des Intelligenzquotienten der Mill Hill-Test und der Progressive Matrices-Test als sehr unzuverlässig. — Die Probleme der Repatriierung wurden kurz diskutiert. Die aus anderen Untersuchungen gewonnenen Erfahrungen lassen vermuten, daß die akute Krankheit und in einem geringeren Maße die Rückfallquote im Anschluß an eine Repatriierung gebessert werden können. Dies darf jedoch nur dann empfohlen werden, wenn man die für den Patienten damit verbundene Belastung vollständig kennt und die Chancen für sein Examen abschätzen kann. Unter diesen Umständen scheint es vernünftig, die Repatriierung solchen Schizophrenen anzuraten, die mehr als 3–4 Monate Klinikaufenthalt benötigen, und jenen, die — unabhängig von der Diagnose — öfter als einmal in eine Klinik aufgenommen werden mußten, da bei ihnen ein Erfolg im Studium unwahrscheinlich ist.
    Notes: Summary The hospital records of sixty West African students admitted between 1955–1965 to a Post-graduate Teaching Hospital were studied. A postal follow-up was then carried out and the records of subsequent admissions were examined. — The problems of diagnosis were examined and the significance of persecutory delusions assessed. They were found to occur in nearly all the patients regardless of diagnosis, and were recognised as being of diagnostic significance for schizophrenia only when they were western in content, either because the persons accused were nonAfrican or because electronic apparatus, etc., was said to be involved. — It was emphasized that feelings of passivity should not be confused with ideas of poisoning and magic. The common occurrence of grandiose delusions in West Africans was confirmed. — Only five of the students qualified. A diagnosis of schizophrenia and more than one admission to hospital were associated with almost certain academic failure. — The Mill Hill and Progressive Matrices were found to be highly unreliable tests for the measurement of intelligence quotient in these students. — The problems of repatriation were briefly discussed. Evidence from elsewhere suggests that the present illness and to a lesser extent the rate of relapse may be improved following repatriation. However, it must be advised with a full knowledge of the stress involved for the patient and a consideration of his chances of passing his examination. — In the circumstances it seems reasonable to recommend repatriation in patients with schizophrenia necessitating more than three to four months in hospital, and those, regardless of diagnosis, who have had more than one admission to hospital, since they appear unlikely to succeed in their studies.

    Notes: The objective of the Health and Ethnicity Project was to identify unmet need and barriers to the use of psychogeriatric and welfare services by ethnic minority older people living in Liverpool's inner-city. The results presented here are the findings of Phase II of the project in which 71 ethnic minority older people aged 65 years and over were interviewed. These respondents had been diagnosed as suffering from dementia or depression earlier in Phase I. They include Black British, Afro Caribbean and Chinese older people. Findings show a low level of service use in spite of considerable unmet need. Only 8% were in institutions and 46% lived alone. For the 17% living alone in sheltered housing, there was evidence of violence towards wardens, wandering, neglect of rooms and occasional fires. Of the 27% living with a family nearly a half of cases also experienced physical disability. Those in institutions were finding language difficulties and problems in obtaining a suitable diet. Using Yeatts et al.'s typology (1992) ‘lack of knowledge’ barriers and ‘lack of intent’ barriers concerned the group as a whole most, i.e. they either did not know of services or perceived them as being culturally inappropriate. Implications for service providers are considerable: to ensure improved access it is necessary not only to invest more in general medical services but to work in partnership with ethnic minority groups to overcome barriers of mistrust and ensure cultural sensitivity in the services offered.

    Notes: Abstract  The tetracycline analogs minocycline and doxycycline are inhibitors of metalloproteinases (MMPs) and have been shown to inhibit angiogenesis in vivo. To further study the mechanism of action of these compounds we tested them in an in vitro model of angiogenesis: aortic sprouting in fibrin gels. Angiogenesis was quantitated in this system by a unique application of planar morphometry. Both compounds were found to potently inhibit angiogenesis in this model. To further characterize the activity of these compounds against MMPs, we determined the IC50s of both compounds against representatives of three classes of metalloproteinases: fibroblast collagenase, stromelysin, and gelatinase A. Doxycycline was found to inhibit collagenase, gelatinase A and stromelysin with IC50s of 452 μM, 56 μM and 32 μM, respectively. Minocycline was found to inhibit only stromelysin in the micromolar range with an IC50 of 290 μM. Since these results suggest that these compounds may not have been inhibiting in vitro angiogenesis by an MMP-dependent mechanism, we decided to test the effects of the potent MMP inhibitor BB-94. This compound failed to inhibit aortic sprouting in fibrin gels, thus strongly suggesting that both doxycycline and minocycline act by an MMP-independent mechanism. These results have implications for the mechanism of action of tetracycline analogs, particularly where they are being considered for the treatment of disorders of extracellular matrix degradation including periodontal disease, arthritis, and tumor angiogenesis.

    Description / Table of Contents: Résumé On a examiné les admissions à St. Francis en 1966 et en 1969–70. 1966 a été la dernière année complète où le service fonctionnait comme division d'observation et 1969–70 la troisième année complète où il fonctionnait comme unité de traitement d'un secteur déterminé. On a fait des comparaisons avec les résultats publiés des années antérieures. On a trouvé que bien qu'il y ait eu des changements dans les diagnostics au cours des années, les états organiques étatient représentés dans une proportion apparemment minimale. Les admissions légales avaient des caractéristiques particulières qui les distinguaient des autres patients.
    Abstract: Zusammenfassung Die Krankengeschichten aller Aufnahmen nach St. Francis im Jahr 1966 und zwischen 1969–70 wurden untersucht. 1966 diente die Einrichtung zum letzten Mal für ein ganzes Jahr als Aufnahmestation und 1960–70 füngierte sie seit drei vollen Jahren als Behandlungseinrichtung für ein definiertes Einzugsgebiet. Mit den veröffentlichten Ergebnissen der früheren Jahre wurden Vergleiche angestellt. Man fand, daß im Zuge diagnostischer Veränderungen über die Jahre hinweg der Anteil an organischen Störungen ein offensichtlich nicht weiter reduzierbares Minimum erreicht hatte. Zwangseingewiesene zeigten besondere Merkmale, die sie von den anderen herkömmlichen Patienten trennten.
    Notes: Summary The case notes of all admissions to St. Francis for the years 1966 and 1969–70 were examined. 1966 was the last complete year that the unit functioned as an observation ward and 1969–70 was the third complete year that it functioned as a treatment unit with a defined catchment area. Comparisons were made with the published findings of previous years. It was found that whereas there had been diagnostic changes over the years, the proportions of organic states had reached an apparent irreducible minimum. Police admissions had special characteristics that set them apart from other formal patients.

    Notes: Summary The paper reviews the main studies using the AGECAT computerised diagnostic system for the diagnosis of dementia in community studies of prevalence and incidence. It is suggested that variations in reported rates may be due to the use of unstandardised methods, and the paper reviews the oldest established standardised interview: the Geriatric Mental State. The AGECAT system is outlined, and the studies of its diagnostic validity indicated. Arguments are presented against the inclusion of cognitive tests in such instruments, and for the critical evaluation of the concepts of mild dementia. The use of AGECAT in the US/UK Diagnostic Project, and in the Liverpool study of Continuing Health in the Community is described. In addition, preliminary information is given on the Liverpool ALPHA study, the MRC UK multi-centre study, studies organised by the Pan American Health Organisation (Argentina, Chile, Cuba), the Eurodem concerted action (based in Rotterdam) and the WHO multi-site studies. Individual studies in Spain and Portugal are also discussed.
