    Description: Objectives: In the absence of unique ID numbers, cancer and other registries in Germany and elsewhere rely on identity data to link records pertaining to the same patient. These data are often encrypted to ensure privacy. Some record linkage errors unavoidably occur. These errors were quantified for the cancer registry of North Rhine Westphalia which uses encrypted identity data. Methods: A sample of records was drawn from the registry, record linkage information was included. In parallel, plain text data for these records were retrieved to generate a gold standard. Record linkage error frequencies in the cancer registry were determined by comparison of the results of the routine linkage with the gold standard. Error rates were projected to larger registries.Results: In the sample studied, the homonym error rate was 0.015%; the synonym error rate was 0.2%. The F-measure was 0.9921. Projection to larger databases indicated that for a realistic development the homonym error rate will be around 1%, the synonym error rate around 2%.Conclusion: Observed error rates are low. This shows that effective methods to standardize and improve the quality of the input data have been implemented. This is crucial to keep error rates low when the registry's database grows. The planned inclusion of unique health insurance numbers is likely to further improve record linkage quality. Cancer registration entirely based on electronic notification of records can process large amounts of data with high quality of record linkage.
    Description: Ziel der Arbeit: Krankheitsregister in Deutschland und einigen anderen Ländern sind darauf angewiesen, mehrere Meldungen zu einem Patienten anhand von Identitätsdaten zusammenzuführen, da keine eindeutige Personenkennung verfügbar ist. Diese Identitätsdaten werden häufig aus Datenschutzgründen pseudonymisiert. Einige Fehler beim Record Linkage sind unvermeidbar. In der vorliegenden Arbeit werden sie für das Epidemiologische Krebsregister Nordrhein-Westfalen, das chiffrierte Identitätsdaten zum Record Linkage verwendet, quantifiziert. Methoden: Eine Stichprobe von Meldungen an das Epidemiologische Krebsregister Nordrhein-Westfalen, die mit der Information über die Zuordnung zu einer Person versehen war, wurde gezogen. Parallel dazu wurden Klartextidentitätsdaten für diese Meldungen durch Dechiffrierung wiedergewonnen, um einen Gold-Standard zu erzeugen. Die Zuordnungsfehlerhäufigkeiten wurden durch Vergleich der Ergebnisse des Routine-Record Linkage mit dem Gold-Standard bestimmt. Die Fehlerraten wurden für umfangreichere Register hochgerechnet.Ergebnisse: In der untersuchten Stichprobe betrug die Homonymfehlerrate 0.015%, die Synonymfehlerrate 0.2%, das F-Maß ergab 0.9921. Eine Hochrechnung auf umfangreichere Datenbanken ergab, dass bei realistischen Annahmen über die Zunahme des Umfangs eine Homonymfehlerrate von etwa 1% und eine Synonymfehlerrate von etwa 2% zu erwarten sind.Schlussfolgerung: Die beobachteten Fehlerraten sind niedrig, darin zeigt sich, dass effektive Methoden zur Standardisierung und Sicherung der Datenqualität implementiert wurden. Dies ist essenziell, damit die Fehlerraten niedrig bleiben, wenn der Umfang des Registers zunimmt. Durch den geplanten Einschluss der eindeutigen Krankenversicherungsnummer ist eine weitere Verbesserung der Güte des Record Linkage zu erwarten. Krebsregistrierung, die allein auf elektronische Meldungen basiert, kann große Datenmengen verarbeiten bei hoher Qualität des Record Linkage.

    Notes: Summary Isocitrate lyase-less (Icl−) mutants were selected from genetically defined strains ofYarrowia lipolytica to investigate the regulation of synthesis of isocitrate lyase (ICL). Eighteen mutations were localized in the geneICLI, which is most probably the structural gene of ICL in this yeast. Atrans-acting positive element (ICL2) could be identified. This gene is not linked toICL1. One mutation was detected in anotherICL gene (ICL3) which is linked to geneICL1. This mutation lowered the ICL activity incis but not intrans position to geneICLI.

    Notes: Abstract Well known sources of contamination are the laboratory environment, vessels, reagents and handling of the sample. Attention is drawn to the importance of cationic impurities included in the ion-exchangers used for trace-matrix separations (TMS) at the sub-ng/g-level. The usual rinsing and conditioning of the resins does not remove all adhering metals and thus leads to contaminated eluates in analytical work. High and badly reproducible blanks are observed, in particular for Na, Mg, Al, K, Cr, Fe, Zn and Ba. They are introduced during the polymerization process and are not ionically bound but obviously included in the polymer matrix. Any time when another “nest” of inclusion is opened in the resin column by mechanical stress, the next irreproducible event of contamination occurs.

    Description / Table of Contents: Summary A 23-year-old patient suffered from episodic angioedema of the face and neck, accompanied by diarrhea and abdominal pain. Additionally, the patient had bronchial asthma, recurrent nasal polyps and allergic rhinoconjunctivitis. Blood examination revealed leucocytosis with eosinophilia. Histological studies showed eosinophilic infiltrates in the skin and the gastrointestinal mucosa. Allergic food reactions and parasites were ruled out. With systemic corticosteroid treatment, the clinical symptoms and the eosinophilia disappeared. This case shows some parallels to previously described syndromes (eosinophilic gastroenteritis, Samter’s syndrome, episodic angioedema with eosinophilia), but to the best of our knowledge this combination of symptoms has not yet been reported.
    Notes: Zusammenfassung Wir beschreiben einen 23jährigen Patienten mit rezidivierenden Angioödemen des Gesichtes und des Halses und begleitender Gastroenteritis. Zusätzlich bestanden ein Asthma bronchiale, eine Polyposis nasi und eine Rhinoconjunctivitis allergica. Neben einer Eosinophilie im Differentialblutbild fanden sich histologisch eosinophile Infiltrate sowohl in den Hautbiopsien als auch in der Ösophagus-, Magen- und Duodenalschleimhaut. Unter systemischer Kortikosteroidtherapie kam es zu einer kompletten Rückbildung der Angioödeme, der Gastroenteritis und der Eosinophilie. Das von uns vorgestellte Krankheitsbild zeigt deutliche Parallelen zu seltenen in der Literatur beschriebenen Syndromen (eosinophile Gastroenteritis, Samter-Syndrom, episodisches Angioödem mit Eosinophilie), ist aber u.W. nach in dieser Symptomenkonstellation bisher noch nicht bekannt.

    Description / Table of Contents: Summary Digital ischmia with gangrene of one or several fingertips has been described as a paraneoplastic syndrome associated with various malignant tumors, especially adenocarcinomas. Most often this paraneoplastic syndrome represents the first symptom of an occult neoplasia in an advanced stage. We present the case of an 83 year-old patient with digital ischemia and gangrenous fingertips in association with a latent adenocarcinoma of the uterus and metastatic involvement of the paraaortal lymph nodes. Acute occurrence of digital ischemia and gangrene without pathological laboratory findings and negative past medical history concerning cardiovascular-induced emboli, arteriosclerotic occlusion, or rheumatologic and autoimmune diseases should suggest this paraneoplastic syndrome and lead to thorough search for an underlying tumor.
    Notes: Zusammenfassung Eine digitale Ischämie mit Gangrän einzelner oder mehrerer Fingerkuppen wurde als seltene Paraneoplasie bei verschiedenen Tumoren, insbesondere metastasierenden Adenokarzinomen unterschiedlicher Organsysteme, beschrieben. Sie kann das erste Symptom einer bis dahin latenten malignen Neoplasie in fortgeschrittenem Tumorstadium sein. Die Befunde dieses paraneoplastischen Syndroms werden anhand der Kasuistik einer 83jährigen Patientin mit einem paraaortal lymphogen metastasierenden latenten Adenokarzinom des Endometrium uteri vorgestellt. Das akute Auftreten einer digitalen Ischämie mit Gangrän bei fehlenden pathologischen Laborbefunden und negativer Anamnese für rheumatologische, autoimmunologische und kardiovaskuläre Grunderkrankungen (i.e. Emboliequelle oder arteriosklerotisch bedingte Verschlüsse) ist charakteristisch für dieses seltene paraneoplastische Syndrom und muß zu umfassender Tumorsuche Anlaß geben.
