    Notes: Summary Disorders and complaints in the neck and shoulder regions are common among industrial workers and are often attributed to occupational musculoskeletal stress. The possible pathophysiological mechanism of occupational stress on the neck and shoulders is reviewed. A mechanical origin for cervical disc degeneration and osteoarthrosis is reported for a few occupational groups. However, a mechanical origin for osteoarthrosis is debatable. A work posture involving elevated arms may accelerate degeneration of shoulder tendons through impairment of circulation due to static tension and humeral compression against the coracoacromial arch. Furthermore, work tasks with repetitive arm movements may evoke shoulder tendinitis or tendovaginitis, probably due to friction. Three possible routes to neck-shoulder muscular pain are discussed; mechanical failure, local ischemia and energy metabolism disturbance.

    Notes: Summary In a plastic boat company we studied workers' attitudes toward wearing respiratory protective equipment and differences in styrene exposure received with and without respirators. The workers studied used either half-facepiece air-purifying or full-face air-supplied respirators as much as possible during the first day of the study. On the second day respirators were used only for short periods or not at all. Individual styrene exposures were measured by personal air sampling in the breathing zone. When using respirators the exposure was measured both inside and outside the respirators. The styrene metabolites mandelic and phenylglyoxylic acids were determined in urine samples collected during the workday. The eleven workers studied used the respirators 52% of the time on the first day and 7% of the time on the second. The reasons for not wearing respirators were that they delayed work, were too tight and uncomfortable, made it difficult to breath, and/or became too warm. The use of respirators during work operations such as spraying, laminating, and painting reduced the styrene exposure by 56%–92% . The excretion rate of mandelic and phenylglyoxylic acids in urine collected at the end of the working day was 30%–99% lower when respirators were worn than when they were not.

    Notes: Abstract The relationship between individual factors, physical and psychosocial exposure at work, and musculoskeletal symptoms in the neck, shoulders, low back, hands, and knees was studied among female nursing personnel working at a Swedish hospital. The personnel had participated in a course in work technique (patient transfer and handling principles). Prior to the course, the subjects had filled in a questionnaire (n = 688). The aim of this cross-sectional study was to elucidate whether different individual and work factors are related to musculoskeletal symptoms in a specific body region. Due to the cross-sectional design, however, causality cannot be discussed. Univariate analyses and multiple logistic regression analyses were performed and yielded similar results. The latter analyses showed that in the present hospital setting, individual factors together with physical and psychosocial work factors were related to symptoms in the neck, low back, and hands; individual factors and psychosocial work factors were related to symptoms in the shoulders; while only individual factors were related to symptoms in the knees. The results of the present study showed that various individual factors and physical and psychosocial work factors were related to musculoskeletal symptoms in the different body regions. Thus, the identification of risk factors might have far-reaching implications for the way in which effective health programs for prevention should be designed in the hospital setting.

    Notes: Background.  The work technique used by health care professionals in patient transfer tasks affects the musculoskeletal load on the professionals, but probably also the safety and well-being of patients being transferred; it is thus a matter of quality of care.Aims.  The aim of this paper is to report a study exploring the relations between the work technique of nurses in patient transfer tasks, and patients’ perceptions of safety and comfort during the transfers.Methods.  The work technique used by 102 nurses at orthopaedic wards to perform two common patient transfers: one transfer higher up in bed and one from bed to wheelchair, were examined using video recordings and an observation instrument. A work technique score for each performed transfer was calculated, indicating the level of musculoskeletal safety for the nurse. Nurses assessed their own work technique and patients rated the perceived safety and comfort on bipolar scales directly after each transfer.Results.  Patients’ perceptions of safety and comfort were positively correlated to the work technique score in both transfers. Patients felt safer and more comfortable during transfers performed with a safe technique, according to the work technique score, than during those performed with a poor technique. Patients’ ratings of safety in the transfer from bed to wheelchair, and their ratings of comfort in both transfers, were positively correlated to nurses’ assessments of their own work technique. However, the correlation coefficients were rather low.Conclusions.  The results support the existence of a relationship between nurses’ skills in patient transfers and quality of patient care.

    Notes: Grating coupled surface emitting lasers with rectangular as well as blazed grating outcouplers were investigated. Horizontal cavity InGaAs/AlGaAs laser oscillators, with two rectangular first order surface gratings as feedback elements, were integrated with single detuned rectangular or blazed second order gratings as surface output couplers, using electron beam lithography and chemically assisted ion beam etching. Superior differential quantum and surface emission efficiencies of 56% and 84%, respectively, were measured for lasers with optimized blazed output couplers. © 1995 American Institute of Physics.

    Notes: Summary It was observed that some patients with glomerulonephritis or interstitial nephritis were welders. A hypothesis that welding fumes were an occupational hazard was put forward and initiated the following case-referent study. 143 cases were defined as living males, age 25 to 65 years, with renal disease which caused renal biopsy from 1976–1982. Three population-based referents for each case were selected from the national register of the total population matched by sex, age and community. Exposure was assessd by means of a questionnaire. Only 7% of the cases and 12% of the referents did not answer the questionnaire. There were no differences in exposure rates between cases and referents, even if exposure restrictions were applied. The power of the case-referent study was high.

    Notes: Summary The dose-response relationship between vibration exposure and vascular disorders in the hands was examined in platers. The study was based on a cross section of 89 platers and 61 office workers divided according to exposure to vibration into four groups. Vibration exposure was assessed by measuring the acceleration intensity on a sample of tools, together with both subjective rating and objective measurements of the exposure time. The frequency-weighted energy equivalent acceleration for 4 h was 4.6–4.7 m/s2. The point prevalence of white fingers was 42% for the plater category currently exposed with an odds ratio of 85. The time laps before contraction of white fingers (latency time) was four years for the 10th percentile, and was shorter than predicted according to the ISO-5349 standard. The prevalence of white finger symptoms staged according to the Taylor-Pelmear scale was comparable to the prevalences according to the Stockholm Workshop Scale. Vibration exposure was the dominant source of white fingers and each year of vibration exposure increased the odds ratio for white fingers by 11%. Distal circulation in the hands was assessed by a timed Allen test. The odds ratio for a positive Allen test was higher for the workers exposed to vibration compared to the non-exposed workers. The use of the timed Allen test is suggested in the clinical examination for vibration white fingers. The observed high risk for contracting white fingers could be prevented by exposure level reduction and/or restriction of exposure duration.

    Notes: Abstract  Hand and arm symptoms among workers using impact and non-impact hand-held power tools were investigated in a cross-sectional study and a 5-year follow-up study. The study population consisted of concrete workers (n=103), truck assemblers (n=234), electricians (n=101), platers (n=140) and lumberjacks (n=102). Of the original 680 subjects, we followed up 312 after 5 years. A questionnaire concerning ongoing hand and arm symptoms, daily exposure to hand-held power tools, type of tool used, and individual factors was administered. More workers using low-frequency impact tools than workers using non-impact tools reported symptoms in the elbows and shoulders. Elbow symptoms were accentuated in the cross-sectional study, while shoulder symptoms were accentuated in the follow-up study. Wrist symptoms were reported by more of those working with high-frequency impact tools than of those using only non-impact tools when the analyses were controlled for age, years in the occupation and smoking habits. A possible explanation of the results found in this study is that low-frequency impact vibration is transmitted to the upper arm, and thus the elbow and shoulder are at risk, while high-frequency impact vibration is attenuated in the hand and wrist and may predominantly cause symptoms there.

    Notes: Summary Serum creatine kinase (SCK) was measured in ten subjects in the laboratory before and after the performance of bicycle ergometry and a lifting task. SCK was significantly increased 24 h and 48 h after the lifting work but not after the bicycle ergometry, although the work performed on the latter was four-times as great as on the former. The lifting work resulted in muscle pain and tenderness and, for six subjects, in clinical signs of shoulder tendinitis. In a field study, an increase in SCK was noted among assemblers/welders and cash-register operators, but not among controllers and forklift-truck drivers. A health interview revealed that muscuao-skeletal complaints were most often located in the upper extremity in the cases of the assemblers/welders and the cash-register operators. It is proposed that the SCK increase during work is a result of a high local muscular load due to fatigue and energy depletion of muscle cells producing a greater efflux of muscle enzymes. The evaluation of SCK changes during work may be an important tool in occupational health for early detection of work tasks producing local muscular strain.
