    Description / Table of Contents: Summary The basic idea of the Collaborative Family Health Care is nothing new. It has its roots in the bio-psycho-social model of medicine developed by George Engel in the U.S.A. and it has a long tradition in the German psychosomatic medicine. At a time of basic changes in our health care system caused by cost considerations owing to the cutting back of resources it is important to note, that Collaborative Family Health Care combines aspects of theoretical models and pragmatic action-orientation. The basic is a systemic view of illness and health. The doctor-patient-dyad is in this view enlarged to a treatment-triad composed of the therapist, the patient and his family, to be able to bundle the whole potential at disposal, i.e. the treatment motivation of the patient, the support of the family and the expertize of the therapist. A lot of studies support the resource- saving potential of coopertive methods. However, their realization is not possible without considerable learning processes of physicians, psychotherapists and other professionals. A potential has to be mobilized, which affects by no means only the costs. The Collaborative Family Health Care offers new challenges to the professional culture of psychotherapists and new chances for influencing the world of medical health care. At a time of obviously reduced professional solidarity, the iniitiatives of CFHC offer a possibility for physicians, patients and their relatives to reach contentment through cooperation. Interested colleagues are invited to collaborate in the German Section of the CFHC.
    Notes: Zusammenfassung Die methodische und institutionelle Distanz zwischen somatischer und psychotherapeutischer Medizin wird derzeit in den USA angesichts umfassender Veränderungen im Gesundheitswesen grundlegend infrage gestellt. Nicht zuletzt als Antwort auf die radikale Markt- und Wettbewerbsorientierung der Managed Care Konzepte gewinnt in der ambulanten und stationären Versorgung das kooperative Behandlungsmodell der Systemischen Familienmedizin (Collaborative Family Health Care) zunehmend an Bedeutung. Ziel der Systemischen Familienmedizin ist die integrierte bio-psycho-soziale Gesamtbehandlung körperlicher und psychischer Beschwerden des Patienten unter Einbeziehung seiner Familie. Eckpunkte dieses Behandlungskonzepts sind eine intensivierte Kooperation der Behandler untereinander und mit der Familie des Patienten sowie die konsequente Ressourcenaktivierung im primären sozialen Netz. Für eine solchermaßen verdichtete Kooperation zwischen somatischen, psychotherapeutischen und psychosozialen Behandlern liegt inzwischen ein ermutigendes Erfahrungswissen vor. Grundhaltungen und therapeutische Techniken der Psychotherapie, insbesondere der systemischen Psychotherapie, lassen sich in diesem Arbeitsfeld gut nutzen. Die verstärkte Integration von Psychotherapeuten in die medizinische Versorgung modifiziert allerdings deren eigene Professionskultur im Umgang mit Zeit, Raum und Kooperation, schafft jedoch zugleich Chancen, die Kultur des Medizinbetriebs zu beeinflussen.

    Notes: The intracellular mechanisms underlying the action of the endogenous vasodilators such as NO/EDRF, adenosine, and prostacyclin acting through cGMP and cAMP, respectively, are not well understood. One important action of cyclic nucleotides in smooth muscle relaxation is to lower the cytosolic Ca2+ concentration by enhanced sequestration into the sarcoplasmic reticulum. The present study was undertaken to elucidate the potential role of phosphorylation of phospholamban, the regulator of sarcoplasmic reticulum Ca2+ pump, for the control of coronary vascular tone by NO/EDRF, adenosine, and prostacyclin. Phospholamban was identified in pig coronary artery preparations by immunofluorescence microscopy, Western blotting and in vitro phosphorylation. Segments of pig coronary artery, with either intact or denuded endothelium, were precontracted with prostaglandin F2α (PGF2α). In endothelium-denuded preparations 3-morpholinosydnonimine (SIN-1), 5′-N-ethylcarboxiamidoadenosine (NECA), and iloprost (ILO) caused both relaxation and phospholamban phosphorylation with the potency: SIN-1 〉 NECA 〉 ILO. The regulatory myosin light chain was significantly dephosphorylated only by SIN-1. In endothelium-intact pig coronary artery, L-NAME caused additional vasoconstriction and a decrease in phospholamban phosphorylation, while phosphorylation of myosin light chain remained unchanged. An inverse relationship between phospholamban phosphorylation and vessel tone was obtained. Our findings demonstrate significant phospholamban phosphorylation during coronary artery relaxation evoked by NO, prostacyclin, and adenosine receptor activation. Because of the close correlation between phosphorylation of phospholamban and vessel relaxation, we propose that phospholamban phosphorylation is an important mechanism by which endogenous vasodilators, especially endothelial NO/EDRF, control coronary vascular smooth muscle tone.
