    Description / Table of Contents: Summary A report is given on a technique for detection of specific IgM antibodies against Treponema. Using an FITC-labelled antihuman IgM serum from rabbits the antibodies are identified in the patient's serum by fluorescence either directly or after gel-diffusion of the serum through sephadex G 200. The findings in serum samples from 609 patients with either untreated or adequately treated syphilis are presented. These show that the method is suitable for the early recognition of a Treponema infection. It is also pointed out that the results of the test would appear to enable the assessment of the effects of a specific antibiotic therapy of the infection.
    Notes: Zusammenfassung Es wird über ein Verfahren zum Nachweis treponemen-spezifischer IgM-Antikörper berichtet. Dabei werden die Antikörper unter Verwendung eines FITC-markierten Anti-Human-IgM-Serums vom Kaninchen im Patientenserum direkt — oder nach Gel-Diffusion des Serums über Sephadex G 200 — fluoreszenz-optisch erfaßt. Die an den Serumproben von 609 Patienten mit unbehandelter bzw. ausreichend behandelter Syphilis erhobenen Befunde werden mitgeteilt. Sie zeigen, daß die Methodik zur Früherfassung einer Treponemen-Infektion beim Erwachsenen geeignet ist. Darüber hinaus wird dargestellt, daß das Testergebnis die Beurteilung des Effektes einer spezifischen, antibiotischen Behandlung der Infektion zu ermöglichen scheint.

    Description: Hintergrund: Für den Hörerwerb ist eine sensitive Periode bis zum Alter von 6 Lebensmonaten beschrieben. Vor dem Hintergrund einer außergewöhnlichen Plastizität des sich entwickelnden menschlichen Gehirns können frühzeitige Beschallung oder elektrische Stimulation ein akustisches Defizit ausgleichen. Material und Methoden: Das universelle Neugeborenenhörscreening in Hamburg wird seit 2002 intensiv interdisziplinär verfolgt. Das Konzept des Hamburger Arbeitskreises Hörscreening bei Neugeborenen war es, von Anfang an zuerst den fachärztlichen Konsens aller beteiligten Facharztgruppen zu suchen. Ergebnisse August 2002 bis Juli 2006: Die Erfassungsrate betrug 63.459 Neugeborene bei einer Rate von 65.466 Geburten. Primär gescreent wurden 93% der Neugeborenen. Die Anzahl der Testauffälligen im Primärscreening lag bei 3,3%. 118 Kinder wurden als schwerhörig diagnostiziert. 74 Kinder wurden mit Hörgeräten versorgt, 6 Kinder erhielten ein Cochlea Implantat. Die Lost-to-follow-up-Rate bei im Primärscreening auffälligen Kindern betrug 31,3%, bei im ambulanten Nachscreening auffälligen Kindern 30,2%. Schlussfolgerung : Zur Verringerung des "Lost-to-follow-up" bedarf es eines namentlichen Trackings unter Berücksichtigung datenschutzrechtlicher Belange und einer speziell hierfür möglichst regional eingerichteten Trackingzentrale.

    Description: Introduction: The study of the visco-elastic characteristics of the vocal fold tissue is important for understanding the biomechanical properties of this tissue and their effect on vocal fold oscillation. The set goal of this work was to determine directional variations of the elastic properties of the vocal fold. Materials and methods : 14 excised human larynges were split along the sagittal plane, mounted without tension, and measured each semilarynx using a Linear Skin Rheometer (LSR). The LSR-probe was attached primarily to the vocal fold epithelium mid-membranous between the anterior commissure and vocal process using a round suction catheter of 2.7 mm outer diameter. The elastic response was measured by applying stress at 5 different angles. Additional measurements were made in a similar procedure for epithelium, lamina propria, ligament and muscle. The successive exposure of different layers was correlated with histology. Results: The distribution of elastic pliability in different directions will be demonstrated. Conclusion: The range of shear modulus derived using this method is comparable to that obtained by our team in-vitro in a previous published study and by other research groups obtained in-vivo or in-vitro. In addition to the confirmation of previous results we highlight an anisotropic nature of the vocal fold tissue.

    Description: Introduction: As in other disciplines, the burgeoning knowledge in ENT medicine long ago surpassed our ability to adequately absorb it and maintain a proper overview. This can give rise to actual or assumed evidence gaps that can impede the progress of the discipline and evidence-based treatment of patients. Clinics and medical practices also hold to traditional doctrines that shape day-to-day medicine, without these schools being challenged based on evidence.Methods: Between February and June 2015, 160 ENT clinics, including 34 university hospitals, and 2,670 ENT practices took part in a two-arm online survey on existing or perceived evidentiary gaps in ENT medicine using a previously developed questionnaire. The survey used for half of the participants was open in form; the other half were given a closed survey with systematics of the field for orientation. The survey was augmented with additional data such as the number of publications and focus areas in the clinics and the age and type of practice of the established physicians.Results: The return rate from the clinics was 39.7%; the return rate of the closed surveys was 29.3%. Of the physicians in medical practice, 14.6% responded to the closed and 18.6% to the open survey. There were no major differences between the two forms of survey. Otological and oncological issues comprised approximately 30% of the list of answers from clinics. Corresponding questions were formulated regarding the current diagnostic and therapeutic problems, such as with stage-related tumor treatment or implantable hearing aids. Diagnostic procedures, e.g., special new procedures in audiology and vestibulogy, dominated the surveys from the practices. However clinics and practices alike cited marginal areas of the discipline that are of daily relevance.Discussion: The cited evidence gaps then needed to be verified or refuted and clarified based on research of the literature as to whether the existing evidence actually reached healthcare providers in the form of guidelines, publications, conferences, or continuing training for application in daily practice. Other steps would include prioritizing future research, evidence mapping, deciding on further systematic reviews, and targeted studies in conjunction with procuring third-party funding and in cooperation with patient associations. The knowledge thus gained should ultimately be transferred in improved form for application in daily clinical practice. Ten questions of key importance each needed to be formulated for the hospitals and practices.
