    Abstract: To identify genomic alterations in chronic lymphocytic leukemia (CLL), we performed single-nucleotide polymorphism-array analysis using Affymetrix Version 6.0 on 353 samples from untreated patients entered in the CLL8 treatment trial. Based on paired-sample analysis (n = 144), a mean of 1.8 copy number alterations per patient were identified; approximately 60% of patients carried no copy number alterations other than those detected by fluorescence in situ hybridization analysis. Copy-neutral loss-of-heterozygosity was detected in 6% of CLL patients and was found most frequently on 13q, 17p, and 11q. Minimally deleted regions were refined on 13q14 (deleted in 61% of patients) to the DLEU1 and DLEU2 genes, on 11q22.3 (27% of patients) to ATM, on 2p16.1-2p15 (gained in 7% of patients) to a 1.9-Mb fragment containing 9 genes, and on 8q24.21 (5% of patients) to a segment 486 kb proximal to the MYC locus. 13q deletions exhibited proximal and distal breakpoint cluster regions. Among the most common novel lesions were deletions at 15q15.1 (4% of patients), with the smallest deletion (70.48 kb) found in the MGA locus. Sequence analysis of MGA in 59 samples revealed a truncating mutation in one CLL patient lacking a 15q deletion. MNT at 17p13.3, which in addition to MGA and MYC encodes for the network of MAX-interacting proteins, was also deleted recurrently.

    Notes: Summary The localization of endogenous peroxidase was studied in the glandula orbitalis (lacrimalis) externa of the rat by the method of Graham and Karnovsky (1966). Reaction product is visible in all cisternae of the rough endoplasmic reticulum including the perinuclear cisternae, in condensing vacuoles, and in all secretion granules. The Golgi cisternae seldom are peroxidase positive. Intercalated duct cells rarely contain reaction product in a few scattered cisternae of the rough endoplasmic reticulum and in secretion granules. After the injection of beef liver catalase reaction product is found in the capillary lumen. Both the injected catalase and the endogenous peroxidase are completely inhibited by 10−2M aminotriazole, while the pseudoperoxidatic activity within the erythrocytes persists. After injection of horseradish-peroxidase reaction product is visible within the capillary lumen and also in the intercellular spaces between lacrimal gland cells. 10−2M aminotriazole completely inhibits the endogenous peroxidase while the exogenous horseradish-peroxidase remains unaffected. The inhibitory effect of aminotriazole is not specific for catalase since lacrimal gland peroxidase is also inhibited.

    Description / Table of Contents: Zusammenfassung Die Struktur der Leberzellen wurde an der Maus nach subcutaner Injektion von 10 γ Phalloidin/g Körpergewicht und an der isolierten, durchströmten Leber von Ratten nach Zusatz von 20 mg Phalloidin zur Perfusionslösung zu Zeitpunkten zwischen 10 und 100 min nach der Vergiftung elektronenmikroskopisch untersucht. Die Strukturveränderungen sind in beiden Versuchsanordnungen gleich. Die Vergiftung führt über eine zunehmende Erweiterung der Zisternen des endoplasmatischen Reticulums zum Auftreten großer, fettfreier, fibrinhaltiger Vacuolen, die schließlich alle Leberzellen durchsetzen. Etwas später finden sich autolytische Vacuolen, in denen Cytoplasmabestandteile eingeschlossen werden. In der Umgebung autolytischer Vacuolen treten herdförmige Cytoplasmanekrosen auf, die teilweise durch eine Membran abgegrenzt werden. Die Kernund Mitochondrienstruktur bleibt intakt. Die Strukturveränderungen sind nicht spezifisch für die Phalloidinwirkung. Die peribiliären Lysosomen sind an der Vacuolisierung des endoplasmatischen Reticulums nicht beteiligt; umschriebene Cytoplasmanekrosen in der Umgebung autolytischer Vacuolen könnten jedoch durch einen Austritt von Hydrolasen nach Schädigung der Membran der autolytischen Vacuolen verursacht sein.
    Notes: Summary The fine structure of liver cells was investigated (a) in mice at time points between 10 and 90 mins after subcutaneous injection of 10 γ phalloidin/g body weight, and (b) in the isolated perfused livers of rats at time points between 10 and 100 mins after addition of 20 mg phalloidin to the perfusate. The structural alterations were identical in both experiments. The cisternae of the endoplasmic reticulum were dilated in the early stages of intoxication and later turned into large vacuoles which were free of fat but contained fibrin; in the later stages all cells became vacuolated. Autolytic vacuoles sequestering cytoplasmic constituents appeared later than the changes of the reticulum. Foci of cytoplasmic necrosis, partially surrounded by a membrane, were found in the vicinity of autolytic vacuoles. The structure of nuclei and mitochondria remained unaltered. The structural changes were not specific for phalloidin poisoning. Peribiliary lysosomes were not involved in the vacuolization of the endoplasmic reticulum. Foci of cytoplasmic necrosis in the vicinity of autolytic vacuoles could have been caused by leakage of hydrolases through the damaged membrane of the autolytic vacuoles.

    Notes: Summary Nonsenescence is a delayed leaf and plant death resistance mechanism in sorghum that circumvents the detrimental effects of reduced soil moisture combined with high temperatures during post-anthesis growth. This drought-tolerance mechanism is often equated with charcoal rot resistance, a widespread root and stalk disease of great destructive potential. Therefore, the inheritance of charcoal rot resistance was investigated directly, by exposure of sorghum to Macrophomina phaseolina, the causal organism, and indirectly, by determination of the inheritance of nonsenescence. Sorghum families derived from diallel crosses between two nonsenescent, resistant inbreds (B35, SC599-11E) and two senescent, susceptible inbreds (BTx378, BTx623) were evaluated in 1989 at College Station and at Lubbock, Texas, under controlled and field conditions. We determined that nonsenescence was regulated by dominant and recessive epistatic interactions between two nonsenescence-inducing loci and a third locus with modifying effects. The same conclusion was reached for charcoal rot resistance. The presence of different genetic mechanisms within SC599-11E for nonsenescence and charcoal rot resistance verifies that these two forms of resistance are not different manifestations of a single trait, i.e., they are not to be equated with each other. We conclude that nonsenescence alone cannot account for, and should not be used as the sole breeding criterion for, resistance to charcoal rot in sorghum.

    Notes: Zusammenfassung Bericht über einen Fall von angeborener Leukämie vom Typ einer Paramyeloblastose mit tumorförmigen Hautinfiltraten, bei dem sich in der Ascendenz eine bösartige Geschwulst fand. Eine kritische und möglichst vollständige Sichtung der Literatur ergibt 13 sicher angeborene und 9 wahrscheinlich angeborene Fälle. In 21 Fällen bleibt der intrauterine Beginn zweifelhaft, sie sind aber zum mindesten als sehr frühzeitig aufgetretene Leukämien zu werten. 10 Fälle sind ganz unsicher und zum größten Teil Erythroblastosen. Mit dem eigenen Fall finden sich in den ersten 3 Gruppen 35 Myelosen und 7 Lymphadenosen, von denen 26 das männliche und 15 das weibliche Geschlecht betreffen.

    Notes: Summary The purpose of this study was to evaluate the indications and results of endoscopic retrograde cholangiopancreatography (ERCP) for gallstone disease since the advent of laparoscopic cholecystectomy. In our personal series of 410 consecutive cases of laparoscopic cholecystectomy, we found 17 common bile duct (CBD) stones; seven were identified by preop ERCP, nine at laparoscopy by intraoperative cholangiography, and one postop by ERCP. We have performed preop ERCP in 21 patients (5.1%); CBD stones were found in seven. Our indications for preop ERCP were elevated liver function tests, dilatation of the common duct by ultrasound, or a history of jaundice/pancreatitis, and all stones were successfully removed by endoscopic sphincterotomy. At laparoscopic cholecystectomy nine patients were found to have stones; one was treated with laparoscopic methods, four with open CBD exploration, and four by postop endoscopic sphincterotomy. Post-laparoscopic cholecystectomy, five patients underwent ERCP for pain or increased liver function tests suggestive of common duct stones. One of the five was found to have stones and these were successfully removed by endoscopic sphincterotomy. ERCP is very useful as a diagnostic and therapeutic modality in laparoscopic cholecystectomy patients with suspected CBD stones. Elevated liver function tests and dilated CBD by ultrasound are the most accurate predictors of stones. Endoscopic sphincterotomy is a more effective route, at present, for stone removal than a laparoscopic approach.

    Notes: Summary Acute cholecystitis is a serious condition in transplant patients and elective cholecystectomy is generally recommended when gallstones are found. We reviewed the results of laparoscopic cholecystectomy (LC) in 10 immunosuppressed transplant patients (6 heart, 4 kidney) and compared them to the results of open cholecystectomy performed in 26 transplant patients (14 heart, 11 kidney, 1 kidney/pancreas). The LC group had a 20% incidence of minor complication with no major complications and no deaths. The open-cholecystectomy group experienced 19% minor complications, 23% major complications, and 15% deaths. The average postoperative length of stay for the LC patients was 4.6 days (2 days for the 5 straightforward cases) as compared to 9.1 days after open cholecystectomy (4 days for the 13 straightforward open cases). Oral immunosuppression was stopped prior to operation but could be restarted within 29 hours after operation in the LC patients and 68 h in the open cases. The findings at LC were helpful in assessing whether acute cholecystitis and/or choledocholithiasis was the source of fever, liver-function abnormalities, or pancreatitis in these immunosuppressed transplant patients. We conclude that LC can be performed safely in transplant patients, but that in 10–20% of patients, the operation will be converted to an open procedure. The advantages of LC in these patients are a shorter hospitalization and less delay to resumption of preoperative oral immunotherapy than after open cholecystectomy.

    Notes: Zusammenfassung Mit nephrotoxischem Serum vom Kaninchen wurde bei 6 Ratten eine Masugi-Nephritis erzeugt, die zu einer schweren Proteinurie führte. Die Glomerula wurden vom 4.–14. Versuchstag licht- und elektronen-mikroskopisch untersucht. Das Endothel ist in den ersten Versuchstagen beträchtlich geschwollen und zeigt eine Vermehrung der Mitochon-drien und des endoplasmatischen Reticulums. Neben kleinen und größeren, auch gegen das Capillarlumen zu vorspringenden Vacuolen treten Fett- und vermutlich auch Eiweißtropfen im Endothelcytoplasma auf. Die osmiophile Mittelschicht der Basalmembran ist geringgradig verdickt. Die Basalmembran der Glomerulumcapillaren ist nicht aufgesplittert. Die osmiophobe Innenschicht ist stellenweise durch Einlagerung von osmiophilem Material verbreitert. Die Poren-weite des Endothels und die Abstände der Deckzellenfüßchen sind nicht auffällig verändert. Die Deckzellen sind geschwollen und stark verästelt. Im Cytoplasma liegen osmiophile Körper und Vacuolen. über den Ort der Antigen-Antikörper-Reaktion sind keine sicheren Aussagen möglich.

    Notes: Abstract Background: Percutaneous endoscopic gastrostomy (PEG) is a good alternative that provides long-term nutritional support and is associated with minimal morbidity. Methods: During a 24-month period, we studied 54 critically injured patients who underwent early PEG to provide enteral nutritional support. Patients were selected due to the inability to tolerate intake by mouth secondary to multiple associated injuries, especially to the central nervous system. Results: All patients sustained multiple injuries with an average Injury Severity Score of 27. The mean Glasgow Coma Scale at the time of admission was 7 and at the time of the PEG was 10. Eleven patients (20%) had an intracranial pressure (ICP) device, and there was no significant increase in the mean ICP before, during, or after the procedure. In 63% of patients, tube feedings were interrupted for a variety of problems in the 72 h preceding the PEG, and in 70% of patients an average of five radiographs were obtained to document tube position. In 95% of patients, the nutritional goal was achieved within 48 h of PEG placement. There were one immediate and two delayed complications after PEG placement. There were two deaths, neither related to the PEG placement. Conclusions: Early PEG in critically injured patients is a safe and effective method of providing access to the GI tract for nutritional support. In patients with significant brain injuries, adequate sedation and the presence of an ICP monitor help to minimize secondary insults to the brain.
