    Description / Table of Contents: Summary On hand of caloric tests on 202 healthy subjects, the coefficient of correlation between the currently most frequently used intensity parameters was established separately for each of the four irrigations, i.e. between maximum velocity of the slow phase, maximum amplitude, maximum frequency, total number of beats and duration of reaction. The maximum velocity of the slow phase is so closely correlated to the maximum amplitude, and the maximum frequency is so closely correlated to the total number of beats (r=always 〉 0.75) that it is sufficient to merely determine one of the two well correlated parameters in healthy subjects. However, the parameters maximum velocity of the slow phase and maximum amplitude are less closely correlated to the parameters maximum frequency and total number of beats (r=smaller, in part considerably smaller than 0.75). A better correlation exists between the nystagmus parameters nearly always by warm irrigations, as compared to cold ones. The findings speak for the fact that maximum velocity of the slow phase and maximum amplitude on one hand, and maximum frequency and total number of beats on the other, are an expression of various characteristics of the experimental nystagmus. In a consecutive publication, we are going to report in which way the various nystagmic parameters of patients are being changed by different lesions of the vestibular system.
    Notes: Zusammenfassung An den thermischen Prüfungen von 202 Gesunden wurden für jede der vier Spülungen getrennt die Korrelationskoeffizienten zwischen den heute gebräuchlichsten Intensitätsparametern bestimmt, und zwar zwischen max. Geschwindigkeit der langsamen Phase, max. Amplitude, max. Frequenz, Gesamtschlagzahl und Reaktionsdauer. Die max. Geschwindigkeit der langsamen Phase ist mit der max. Amplitude und auch die max. Frequenz ist mit der Gesamtschlagzahl so eng korreliert (r=immer 〉 0,75), daß es zumindest an Gesunden hinreichend ist, jeweils einen der beiden gut miteinander korrelierten Parameter zu bestimmen. Die Parameter max. Geschwindigkeit der langsamen Phase und max. Amplitude sind jedoch mit den Parametern max. Frequenz und Gesamtschlagzahl weniger eng korreliert (r = kleiner, zum Teil erheblich 〈 0,75). Bei den Warmspülungen besteht fast immer eine höhere Korrelation zwischen den Nystagmusparametern als bei den Kaltspülungen. Bei letzteren scheinen in die Stärke der nystagmischen Reizantwort über den thermischen Reiz hinaus in größerem Ausmaß als bei den Warmspülungen weitere Faktoren einzugehen. Die Befunde sprechen dafür, daß max. Geschwindigkeit der langsamen Phase und max. Amplitude einerseits und max. Frequenz und Gesamtschlagzahl andererseits Ausdruck verschiedener Eigenschaften des experimentellen Nystagmus darstellen. In einer nachfolgenden Veröffentlichung werden wir darüber berichten, wie die verschiedenen Nystagmusparameter an Patienten durch unterschiedliche Läsionen im Bereich des vestibulären Systems verändert werden.

    Notes: Summary In an earlier investigation (Mulch and Trincker, 1975) it was established that 50% of 120 carefully selected young healthy persons normally showed, when completely excluding any fixation, a slightly intensive vestibular spontaneous nystagmus in at least one position. In a recent series of investigations we now examined 102 test persons from 6 age groups (17 persons per group) between 11 and 70 years of age with the aid of the ENG. Subjectively, these test persons were without complaints. However, contrary to our earlier group of test persons, we did not eliminate those patients with a case history including diseases likely to lead to vestibular damage (e.g. cocussion, otitis). With these investigations we were in the position to prove that the spontaneous nystagmus described in our earlier paper can occur at any age and with the same frequency. Thus, we could exclude that it is the consecutive symptom of an earlier, perhaps traumatic injury (Jatho). If we detected spontaneous nystagmus, its average intensity (speed of the slow phase, total amplitude/min, beats/min) when registered with open eyes in darkness was the same in all age groups. In addition, the present group of test persons was compared with the recent one. These Findings as well as others, which still have to be described, confirm our hypothesis that the here detected spontaneous nystagmus, which is not visible with the Frenzel glasses, is a physiological incident. These findings will have to be considered particularly when giving an opinion on patients with suspected vestibular diseases.

    Notes: Summary It was investigated by nystagmographic recording and simultaneous observation of 117 caloric tests with Frenzel-glasses, of which amount the most important nystagmus parameters (slow phase velocity, frequency, amplitude) must be that nystagmus can be shurely recognized by a trained observer with Frenzel-glasses. Nystagmus of much lower intensity is reliably identified by Frenzel-glasses as presumed till now (Jongkees and Philippszoon 1964). The slow phase velocity of the first clearly recognized nystagmus for example was in 85% of the tests of an intensity of less than 2.5°/s. Usually, it was the first nystagmus at all, which was proved by marking of electronystagmographic recording. In about 30% of the tests, nystagmus of an intensity of 1°/s slow phase velocity and less was recognized. The results suggest a new assessment of the diagnostic value of the examination by Frenzel-glasses compared with nystagmography.

    Notes: Summary In 49 subjects with binaural deafness and 51 subjects with hearing impairment at least bordering on deafness, the question was examined whether there are, in addition to the low-structured tinnitus, highly structured acoustic phenomena after hearing loss analogous to the scenic-optical experiences of subjects with late blindness and the phantom-limb sensation of amputees. Mostly every day or often, 31 of the 100 subjects examined have musical experiences in the form of melodies, songs and concerts, which are usually of extreme forcefulness and can be precisely differentiated. Some of them think they can “discern” individual musical instruments in the total sound of the “music”. The beginning and end cannot be consciously controlled; constant repetitions are typical, as when a record is played over and over again. The phenomena are best classified as pseudohallucinations, since they are recognized — usually right away, but sometimes only after brief reality control — as “inner hearing”. Results are reported from anamnestic, audiological and psychological examinations as well as from tests of communication ability (capacity for speech perception by lipreading), which were carried out in all subjects for clarification of the genesis of this phenomenon.

    Notes: Summary One hundred and twenty subjects were selected from a large number of healthy persons on the grounds of their medical history and preexaminations including audiogram, which excluded the possibility of a previous cochleo-vestibular illness. These 120 healthy persons were examined by means of electronystagmography. We were searching for spontaneous and positional nystagmus in 5 positions with open eyes in darkness and with closed eyes. Fifty percent proved to have a horizontal and 82% a vertical spontaneous nystagmus in at least one position. The sex had no influence on the frequency and there was no difference in results under smokers and non-smokers. There was the same frequency of nystagmus directed to right and left. We conclude that there is a physiological horizontal spontaneous nystagmus of low intensity if fixation is completely excluded. In our opinion it is not possible to determine a certain level of intensity above which a spontaneous nystagmus is pathological as the intensity of nystagmus is essentially dependant upon non-vestibular elements.

    Notes: Summary It was the purpose of this study to investigate to what extent porous synthetics, e.g. porous high density polyethylene, is suitable for otosurgical interventions. In this study, we examined the suitability of this material, in particular for the obliteration of radical cavities and for the replacement of the posterior wall of the auditory meatus on experimental animals. In 54 guinea pigs with infected and non-infected middle ears, the posterior wall of the auditory meatus was removed and then replaced by an implant consisting of high pressure polyethylene. The bulla was filled with synthetic material. After periods lasting from 1–24 weeks, the animals were sacrificed, the synthetic implants, together with the surrounding tissue, were removed and examined macroscopically as well as histologically. The macroscopical examination revealed that almost 80% of the implants had settled well and developed contact to the surrounding tissue; partially even in cases where a massive infection of the middle ear spaces existed. The histological examination showed that as early as 1 week after implantation, the formation of connective tissue was visible in the area of the bulla in all of the animals; after 16 weeks, bone growth was found as well in 80% of the animals. In the area of the posterior wall of the auditory meatus, the ingrowing of tissue appeared to be delayed. However, here, too, infiltrations of connective tissue was found in all of the animals and in 60% of them, the formation of new bone. Generally, there was only a slight reaction in the surrounding region. Since up to now, the period of observation lasted only for 6 months; the results achieved so far, will have to be regarded with some reservations.

    Notes: Summary First the surgical prodecure is shown for establishing the path of access for the endoscope: Antrotomy with visualisation of the sigmoidal sinus, the posterior vertical semicircular canal, the dura of the middle cranial fossa and Trautmann's triangle; then, after removal of the last bony layer above Trautmann's triangle and the sigmoidal sinus, the incision of the dura is carried out. Both the cerebellopontine angle during a diagnostic endoscopy in a case of suspected acusticus neurinoma and the therapeutic endoscopy with selective parapontine neurotomy of sensible branches of the trigeminal nerve in a patient with tic douloureux are then shown. Indications for endoscopy and possible complications are discussed on the basis of the results of 24 endoscopies.

    Notes: Summary Endoscopy of the cerebellopontile angle via Trautmann's triangle has found successful clinical application. Endoscopies were performed by diagnostic indication in three patients. Tumorous, non-significantly space-occupying processes were involved here that could not be satisfactorily clarified by neuroradiological methods. Endoscopy and endoscopic neurotomy were carried out in 13 patients by therapeutic indication in cases of trigeminal or glossopharyngeal neuralgia. In our opinion, the procedure is relatively simple and very effective after sufficient practice with the anatomical specimen. Apart from sensorineural hearing impairment in one patient, which cannot yet be conclusively assessed, serious complications were not observed in any of the patients.

    Description / Table of Contents: Summary The problem of a replacement material for the reconstruction of the posterior wall of the auditory canal, as well as for the obliteration of the radical cavity has not been solved satisfactorily so far. In order to examine the possible applicability of porous plastics, the posterior wall of the auditory canal of guinea pigs was first removed and then reconstructed using porous polyethylene and filling the bulla tympanica with fine polyethylene chips. One third of the animals had an infected middle ear at the time of implantation. Histologic assessments were made until 24 weeks after implantation. In 80% of the animals the implant settled well, even in cases where the middle ears had been originally infected. By proliferation of vessels and connective tissue, the polyethylene had almost been completely integrated into the surrounding tissue after one week. Extent and frequency of metaplastic osseous growth was greater in the chip-filled bulla than in the reconstructed posterior wall (Fig. 9). After four weeks, the new osteoid substance was firmly connected with the regional bone (Fig. 4). According to these results, it would seem possible to use porous polyethylene even for the nonaseptic implant bed as it is found in the chronically infected middle ear.
    Notes: Zusammenfassung Das Problem des Materialersatzes bei Rekonstruktion der hinteren Gehörgangswand bzw. Obliteration der Radikalhöhle ist bisher nicht zufriedenstellend gelöst. Um die mögliche Verwendbarkeit von porösen Kunststoffen zu untersuchen, wurde bei Meerschweinchen die zunächst entfernte Gehörgangswand durch poröses Polyäthylen rekonstruiert und die Bulla tympanica mit Polyäthylenspänen ausgefüllt. Bei einem Drittel der Tiere waren die Mittelohrräume bereits bei Kunststoffimplantation infiziert. Histologische Untersuchung erfolgte bis 24 Wochen nach Implantation. Bei 80% der Tiere heilten die Implantate gut ein, und zwar auch dann, wenn bereits primär infizierte Mittelohrverhältnisse vorgelegen hatten. Durch Einsprossen von Gefäßen und Bindegewebe war der Kunststoff nach einer Woche weitgehend in den unigebenden Gewebsverband integriert. Ausmaß und Häufigkeit von metaplastischem Knochenwachstum war in der mit Spänen gefüllten Bulla tympanica größer als in der hinteren Gehörgangswand (Abb. 9). Bereits nach vier Wochen stand der neugebildete Knochen in fester Verbindung mit dem ortsständigen Knochen (Abb. 4). Nach den Ergebnissen scheint der Einsatz von porösem Polyäthylen auch in nicht aseptischem Aufnahmelager, wie dem chronisch entzündeten Mittelohr, möglich zu sein.

    Notes: Summary Before we could use our program for automatic on-line analysis of nystagmus in clinical routine we had to make sure correspondence of results from computer and manual evaluation. Therefore we compared maximal values of the slow phase velocity (SPV) and the number of beats in 10 s from thermal tests in 100 patients. Correlation coefficients of 0.96 for the SPV-values and of 0.87 for the number of beats were found. For the relation of responses between left and right ear stimulation the correlation is very high too. All correlation values are significant at 0.1%. These results show, that our automatic analysis of nystagmus can replace the laborious manual evaluation of electronystagmograms, if records are fairly well done. In addition the computer determines the temporal course of SPV as a good aid for the judgement of nystagmus.
