    Description / Table of Contents: Zusammenfassung Es wird ein Untersuchungsgang zur Differenzierung von antraler und vagaler Stimulationsphase am Magen dargelegt. Als Stimulantien werden 1. Betazol und 2. Insulin verwandt. Insulin + Atropin wird bei einem dritten Test eingesetzt. Es wurde gefunden, daß Atropin am Magen maximal 50 min vagolytisch wirkt. Dieser Untersuchungsgang erlaubt bei persistierender HCl-Sekretion nach Magenresektion zu entscheiden, ob eine Vagotomie oder eine Nachresektion indiziert ist.
    Notes: Summary A test method for the differentiation of the antral and vagal stimulation phase of the stomach is presented. Betazol and insulin were used as stimulants. Insulin + atropine is used in a third test. It was found that the maximum vagolytic effect of atropine on the stomach occurs after 50 min. This test method allows a decision in patients with persistent hydrochloric acid secretion after gastric resection whether a vagotomy or a secondary resection is indicated.

In beiden Gruppen konnte der Ausfall eines oder mehrerer Enzyme nachgewiesen werden.
    Notes: Summary In 20 patients with stomach resections accord. to Billroth I and another 20 patients with resections accord. to Billroth II, the exocrine secretory performance of the pancreas was quantitatively examined after 6.4, respectively 6.5 years. In both groups a deficiency of one or more enzymes could be demonstrated.

    Description / Table of Contents: Zusammenfassung Nach kurzen Übersichten über den heutigen Stand der Möglichkeiten, parenteral oder per Sonde zu ernähren, wird über spezielle Probleme der Ernährung bei der Tetanuserkrankung berichtet. Eine optimale Ernährung dieser Kranken mit ausgeglichenen Stickstoffbilanzen und Normalisierung des Elektrolythaushaltes kann erst erreicht werden, wenn die Kombination von parenteraler und Sondenernährung möglich ist. Eine solche Kombination kann aber nur bei intaktem Schluckreflex empfohlen werden.
    Notes: Summary After a brief resume of the possibilities of parenteral or probe-feeding, we report on the special problems of feeding in tetanus. Optimum nourishment of these patients with a balanced nitrogen-exchange and normalisation of the electrolytemetabolism can only be achieved, if combined parenteral and probe-feeding is possible. However, such a combination can only be recommended, if the deglutitionreflex remains intact.

    Description / Table of Contents: Zusammenfassung Es wird die Treffsicherheit der selektiven Arteriographie als Lokalisationsmethode von Inselzelltumoren des Pankreas demonstriert (Tab. 1). Nach den Literaturangaben und den eigenen Ergebnissen gelang der angiographische Nachweis in knapp 80%. Voraussetzung für eine optimale Diagnostik ist die technisch gute Darstellung aller pankreasversorgenden Arterien. Wir ziehen die alternierende Methode der simultanen Arteriographie von Art. coeliaca und Art. mesenterica superior vor. Nach der Injektion von 35 ml 70%igen Conray1 mit dem Druckinjektor (15 ml/sec) werden Serienaufnahmen (3 Bilder/sec) mit Erfassung aller Phasen angefertigt. Sehr kleine Tumoren unter 0,8–1 Ø PI und spärlich vascularisierte Geschwülste mit massiver Hyalin- oder Amyloidablagerung bzw. Fibrosierungen entgehen dem angiographischen Nachweis. Am Beispiel eines extrapankreatischen hypoglykämisierenden Tumors (sekundäres Lebersarkom) wird auf das Krankheitsbild der Tumorhypoglykämie hingewiesen.
    Notes: Summary The reliability of selective arteriography in the localization of islet cell tumours of the pancreas is demonstrated by Table 1. According to the literature and the results obtained by us, angiographic diagnosis is possible in close to 80% of the cases. A prerequisite of optimal diagnostic results is of course the technically satisfactory visualization of all the arteries which supply the pancreas. We favour the method of alternating arteriography of the coeliac and superior mesenteric arteries. After the injection of 35 ml of 70% cone. Conray1 with a pressurized injector (15 ml/sec), series of X-rays (3 pictures per second) are taken to include all phases. Very small tumours (under 0.8–1 cm Ø) and poorly vascularized growths with massive hyaline or amyloid deposits or fibrosis will escape angiographic diagnosis. Using the example of an extra-pancreatic tumour which causes hypoglycaemia (secondary liver sarcoma) the syndrome of tumour-hypoglycaemia is demonstrated.

    Notes: Summary Summarizing we would say: approximately 30% of the total calories of a balanced parenteral regime are assumed at the moment by many workers to be an optimal fat ratio in patients without the immediately preceding influence of stress Very many patients are fed parenterally after operations or trauma. The caloric requirements of these patients are increased. The possibility of covering these requirements, however, by the use of high-caloric fat emulsions is rather limited as is demonstrated by the deterioration of N-balances. One reason may be the combustion of endogenous fat reserves maximally activated after the stress — which of course means an abundant availability of fat at a cellular level. Another reason may be the activation of gluconeogenesis by congested acetyl-CoA as a specific biochemical interrelationship in cellular metabolism. An optimum for the fat ratio in postoperative states is not as yet known. Fat emulsions and ethyl alcohol — in spite of both being valuable highcaloric substrates — are metabolically incompatible both in stressed and unstressed patients.

Solche können als Folgen von Dysregulationen in den Bereichen des Wasser-, Elektrolyt- und Säure-Basen-Haushaltes, der Sauerstoffversorgung der Zellen, des Energie- Bowie des Bau- und Funktionsstoffwechsels (unter besonderer Berücksichtigung des Proteinstoffwechsels) und des Vitamin- und Spurenelementhaushaltes auftreten. Eine schematisierte Synopsis versucht, in diese Thematik einzuführen.
    Notes: Summary A fundamental knowledge of postaggression metabolism, i. e., postoperative and posttraumatic metabolism, is necessary for prophylaxis and treatment of its deleterious dysfunctions. These may be due to disturbed regulation of the water, electrolyte, and acid-base balance, the oxygen supply to cells, energy metabolism as well as structural and functional metabolism (with special emphasis on protein metabolism), and vitamin and trace-element metabolism. This schematic synopsis is a preliminary introduction to this topic.

    Description / Table of Contents: Summary We proved the influence of chelating agents on the content of zinc and the synthesis of glycogen in diaphragms and livers of rats and on their renal zinc excretion. Dithioerythritol (DTE) decreased the synthesis of glycogen in vitro as did dimercaptopropanol (BAL) in vivo in the liver and diaphragm. Whereas the renal excretion of zinc with BAL was increased, the content of zinc in diaphragms with DTE remained unchanged. These observations are in accordance with the assumption that endogenous chelating agents are involved also in the impairment of glucose tolerance after acute stress by masking the zinc necessary for insulin action.
    Notes: Zusammenfassung Der Einfluß von Chelatbildnern auf Zinkgehalt und Glykogensynthese in Zwerchfell und Leber von Ratten sowie deren Zinkausscheidung wurden geprüft. Dithioerythrit verminderte die Glykogensynthese in vitro, Dimerkaptopropanol (BAL) bewirkte dasselbe in Leber und Zwerchfell in vivo. Während unter BAL die renale Zinkausscheidung erhöht war, blieb der Gehalt des Zwerchfelles an Zink unter DTE unverändert. Diese Beobachtungen stehen im Einklang mit der Annahme, daß auch in der Phase eines akuten Stresses endogene Chelatbildner an der Glukosetoleranz-Störung beteiligt sind, indem das notwendige Zink maskiert wird.
