    Abstract: Background/Aims: In adults, nighttime hypertension and hyperuricemia are new risk factors for progression of IgA nephropathy (IgAN). In children, nighttime blood pressure (BP) and serum uric acid have never been investigated. The aim of the study was to investigate nighttime BP and uric acid in children with IgAN. Methods: Data on children with IgAN from two pediatric nephrology centers were retrospectively reviewed (renal biopsy - subclasses according Hass I-V, ambulatory blood pressure monitoring ABPM, serum uric acid, proteinuria). Results: Twenty-eight untreated children with IgAN were included. Hypertension was diagnosed on the basis of ABPM in 54% of children, 50% were nondippers and 25% have isolated nighttime hypertension. The mean ambulatory BP was higher at nighttime than during daytime (systolic nighttime BP 1.11 +/- 0.79 SDS vs. daytime 0.59 +/- 0.79, diastolic nighttime BP 1.16 +/- 0.95 vs. daytime 0.52 +/- 1.10, p 〈 0.01 for systolic and p = 0.01 for diastolic). Children with severe histological subclasses (III-IV) tended to have higher prevalence of hypertension than children with mild subclasses (I-II), 67% vs. 38%, p = 0.13. Hyperuricemia was diagnosed in 14% of children. A significant correlation was found between proteinuria and histopathological subclasses (r = 0.44, p 〈 0.05). Conclusion: Children with IgAN have often nighttime hypertension. Hypertension and proteinuria are associated with severe histopathological findings. Hyperuricemia is a rare finding in children.     Description / Table of Contents: Zusammenfassung Innerhalb von 2 Jahren wurden in die Infektionskinderklinik Brünn 70 Kinder mit infektiöser Mononucleose eingewiesen. Bei diesen Erkrankten bestimmten wir neben den geläufigen Lebertests auch die Aktivität von GOT, GPT, ALD, MDH, LDH und der LDH-Isoenzyme. Bei der Mononucleosis infektiosa (M. i.) finden wir zwei Typen der LDH-Isoenzymogramme. Der erste ist durch das Ansteigen der LDH3- und LDH4-Aktivität bei der typhösen Form charakterisiert. Der zweite ist durch Aktivitätserhöhung der Isoenzyme LDH3, LDH4 und LDH5 bei der anginösen Form und bei der ikterischen Form gekennzeichnet. Dieses kombinierte Isoenzymbild zeugt von einer Leberschädigung sowie gleichzeitig von der Schädigung anderer Organe, vermutlich des lymphoreticulären Systems. Die Schädigung der Leber kann im Verlauf einer M.i. durch eine gleichzeitig auftretende Streptokokkenerkrankung noch verstärkt werden. Für einen Leberschaden ist die Zunahme der langsam wandernden Fraktion charakteristisch, die von dem Isoenzym LDH5 dargestellt wird.
    Notes: Abstract 70 cases of infectious mononucleosis admitted to the Childrens Hospital, Brno, over a period of 2 years have been studied. The investigation of serum glutamic transaminase (SGOT), glutamic pyruvic transaminase (SGPT), aldolase (ALD), malic dehydrogenase (MDH), total lactic dehydrogenase (LDH), LDH-isoenzymes, total bilirubin and thymol turbidity was carried out as routine test. LDH-isoenzyme determination is of value in detecting any involvement of other organs during the course of infectious mononucleosis. Two types of isoenzymograms are obtained from these cases: the first is characterized by an elevated LDH3, LDH4 activity (typhoidal type of infectious mononucleosis); and the second, where a rise in LDH3, LDH4, LDH5 is prevalent (pharyngeal type with liver involvement; icteric type). The overall picture of the isoenzymograms proves the impairment of the liver and other tissues, particularly those of the lymphoreticular system. A streptococcal infection running concurrently with the infectious mononucleosis, may contribute toward an increasing liver impairment. Characteristic for a liver involvement is the elevation of the slow migrating fraction (isoenzyme LDH5).

    Description / Table of Contents: Zusammenfassung Das Verhalten der Lactatdehydrogenaseaktivität und ihrer Isoenzyme wurde bei 42 Kindern mit Mumps untersucht. Die höchsten Serum-LDH-Werte haben wir am 1. und 4. Kliniktag festgestellt. Die Erhöhung der gesamten LDH-Aktivität bei Mumps mit Parotis- oder Glandula submandibularis-Schwellung wurde durch ein Anwachsen der LDH2, evtl. LDH3, bei parotischer Meningoencephalitis durch ein Anwachsen der LDH1 und bei parotischer Orchitis durch eine Erhöhung der LDH5, bedingt. Eine mäßige LDH5-Aktivitätserhöhung fanden wir bei 34 Kindern mit Mumps. Nur bei 8 Kranken handelte es sich gleichzeitig um eine Leberschädigung.
    Notes: Abstract 42 cases of mumps admitted to the Children's Hospital, Brno, were studied with respect to LDH-activity and its isoenzymes. The determination of total serum lactic dehydrogenase (LDH), LDH-isoenzymes, serum glutamic oxaloacetic transaminase (SGOT), and serum glutamic pyruvic transaminase (SGPT), was carried out as a routine test. An increase of the total LDH-activity was demonstrable on the first and fourth day after admission. Three types of serum LDH-isoenzymograms were obtained from these investigations: the first is characterized by an elevated LDH2, LDH3 (in mumps); the second by an elevated LDH1 (in mumps-meningoencephalitis); the third by an elevated LDH5 (in mumps-orchitis). A minimal rise in LDH5 was demonstrable in 34 out of 42 children with mumps, out of which only 8 cases (with a decrease in the SGOT/SGPT ratio to below one) showed a concurrent liver lesion.

    Description / Table of Contents: Zusammenfassung Bei 69 Kindern, die an Varicellen erkrankt waren, wurden wiederholt die Aktivitätswerte der GOT, GPT, LDH, N-Acetyl-β-Glucosaminidase und der LDH-Isoenzyme im Serum gemessen. Eine Leberschädigung wurde bei 12 Kindern am 1. Tag und bei 6 Kindern am 10. Tag des Krankenaufenthaltes nachgewiesen. Die mathematisch-statistische Auswertung der betreffenden Werte ergab, daß die Aktivitätsunterschiede am 1. und am 10. Tag der Hospitalisation im Vergleich zu den Normalwerten, ebenso wie die Aktivitätsunterschiede zwischen dem 1. und dem 10. Tag, statistisch nicht signifikant sind. Es wurden Veränderungen der Lipoproteine und der LDH-Isoenzyme bei einem letal endenden Varicellenverlauf mit Reyschem Syndrom beschrieben.
    Notes: Abstract The activity of GOT, GPT, N-acetyl-beta-glucosaminidase and LDH isoenzymes in the serum were followed up in 69 children suffering from varicella. The liver damage was proved in 12 children on the 1st day of hospitalization and in 6 children on the 10th day of hospitalization. Statistical evaluation of the enzymes followed proved that the differences of between activities recorded on the first and 10th days of hospitalization and the normal values were statistically insignificant, as were the differences between activities on the first and on the 10th day of hospitalization. The changes of in the levels of lipoproteins and LDH-Isoenzymes in Rey's Syndrome with lethal outcome in the course of varicella are demonstrated.
