    Description: The aim of our poster is to present several cases of Myasthenia gravis (MG) patients with head and neck manifestations including ocular ptosis, laryngeal paresis, dysphagia and velopharyngeal insufficiency.Materials and methods: These MG patients were referred to our clinic in 2015 for evaluation on the account of their complaints. Among the used methods were full neurological and ENT examinations, CT scan of neck and mediastinum (one had untrearted thymoma), fibrolaryngoscopy, serological testing for AChR antibodies.Discussion/conclusion: Myasthenia gravis (MG) is a relatively rare autoimmune disorder in which antibodies form against acetylcholine nicotinic postsynaptic receptors at the neuromuscular junction of skeletal muscles. The usual initial complaint is a specific muscle weakness that progresses to the more severe generalized form in most patients, producing severe muscular deterioration. As far as the head and neck region is concerned, most common is extraocular muscle weakness or ptosis ; weakness of the facial muscles is almost always present. Laryngeal involvement manifests with flaccid paresis and dysphonia. Patients can also have dysphagia, hypernasality, and velopharyngeal insufficiency.MG treatment includes medical therapy - anticholinesterase agents, immunosuppresive agents- and surgical thymectomy if a tymoma is present. Our patients were treated with corticosteroids with significant improvement, one of them was referred for surgical treatment.Although rare, MG should always be considered in the differential diagnosis of the neurological disorders of the head and neck region and it has to be distinguished from cranial nerve neuropathies and other infectious and non-infectious diseases that cause dysphonia, dysphagia, etc.Der Erstautor gibt keinen Interessenkonflikt an.

    Description: Primary malignant lymphomas of the salivary glands are relatively rare. Clinical manifestation is not uniform and the disease initially often remains unrecognized. Lymphoma appears as "a surprise" for the surgeon, which leads to unnecessarily extensive surgery with all accompanying risks.Aim: To stress the importance of clinical thinking towards malignant lymphoma of the parotid and review the data from history, presentation and radiological investigations, which may direct us to the diagnosis.Results and discussion: We present a case of a 54 years old female with Parotid NHL, diagnosed after surgery and controlled successfully with radiation and chemotherapy. Suggestive factors, typical CT symptoms and options for histologic sampling are discussed.Conclusions: Primary parotid NHL is a disease with good prognosis and high local tumor control. If it is considered, unnecessarily extensive surgery can often be avoided. Parotidectomy is indicated only in cases when fine needle aspiration biopsy is impossible, no other tumor tissue is accessible for sampling and the results from FNA or frozen sections are unreliable. The role of surgery is purely diagnostic. The proved NHL is treated exclusively conservatively.Der Erstautor gibt keinen Interessenkonflikt an.

    Description: The examination of the Eustachian tube is one of the most important factors for the success of tympanoplasty and treatment of middle ear infections. We propose a prototype of newly developed device for acoustic measurement of Eustachian tube function. This new approach can complement to the standard tests (Valsalva and Toynbee).     Description: Evolution in cochlear electrode design of different manufacturers is discussed. The differences in electrode number, location and stimulation patterns are outlined. The interactions between stimulation mode and speech coding strategies are summarized. Literature data for the outcomes using several coding strategies are compared. Advantages and disadvantages are clarified. Some experimental electrodes are reviewed.Based on the clinical experience with different types of cochlear electrodes the theoretical design of a modiolar electrode is proposed. As its length can be about 7 mm a special design of the electrode array is required. Changes of the shape and configuration of electrode contacts are also necessary.In spite of the continuous progression in cochlear implant technology there is still a broad field for investigation in the direction towards the achievement of a more physiological hearing.
