    Description / Table of Contents: Summary We report the case of an 8 year old boy with a rare congenital myopathy (with cytoplasmic inclusions bodies) and clinical signs of rigid-spine-syndrome. During treatment of lung tuberculosis disordered breathing with hypoventilation during REM-sleep, hypoxemia and persistent hypercapnia during daytime were found. This complication in myopathies is caused by weakness of respiratory muscles and leads to negative features (like pulmonary hypertension, daytime-sleepiness, growth failure or further impairement of the muscle disease). Diagnostic steps in disorders of breathing during sleep in children with neuromuscular disease are shown. Conclusions: patients with congenital myopathies should be checked for impairment of respiratory muscle function and disordered breathing during sleep. Nasal intermittent positive pressure ventilation (NIPPV) is an effective and non-invasive treatment for the prevention of nocturnal hypoxemias in these patients and may improve life quality considerably.
    Notes: Zusammenfassung Es wird über einen 8jährigen Jungen mit einer seltenen kongenitalen Myopathie (myofibrilläre Myopathie mit zytoplasmatischen Einschlußkörperchen) und dem klinischen Bild eines Rigid-spine-Syndroms berichtet, bei dem im Rahmen der Behandlung einer Lungentuberkulose eine nächtliche Atemstörung mit REM-assoziierten Hypoventilationen, Hypoxämien und persistierender Hyperkapnie diagnostiziert wurde. Diese bei Myopathien vorkommende Komplikation ist Ausdruck einer Störung der Atempumpe; sie kann zahlreiche negative Auswirkungen nach sich ziehen (u.a. pulmonale Hypertonie, Tagesmüdigkeit, Wachstumsverzögerung, Gedeihstörung und weitere Verschlechterung der muskulären Insuffizienz). Exemplarisch werden die Schritte zur Diagnose einer schlafbezogenen Atemstörung bei muskelkranken Kindern aufgezeigt. Schlußfolgerung: Bei angeborenen Myopathien sollte sorgfältig nach Störungen der muskulären Atempumpe und schlafbezogenen Atemstörungen gefahndet werden. Eine intermittierende nasale Maskenbeatmung stellt eine effektive und nichtinvasive Behandlungsmöglichkeit nächtlicher Hypoventilationssyndrome dar und kann die Lebensqualität betroffener Patienten erheblich verbessern.

    Notes: The peak profiles of standard reference materials have been investigated with energy-dispersive X-ray diffraction at a synchrotron source. Keeping the detector dead time below 2%, the peak profiles were essentially pure Gaussians independent of energy in the range investigated. Plastic deformation of ductile f.c.c.-based materials (Cu3Au and Au) leads to increased Lorentzian contributions to the peak profiles. A single-line profile analysis was performed to obtain estimates of size and strain; reasonable values were found.

    Notes: The compression behaviour of a foil of Cu3Au in a pressure medium of NaCl has been studied by energy-dispersive X-ray diffraction in a diamond-anvil cell. Evidence from stress analysis and peak broadening of the foil (the most extreme example of a non-ideal powder) throws light on the compression of powders under non-hydrostatic conditions. A complete pressure cycle, including re-pressurization after pressure release, shows that significant plastic deformation takes place which results in large deviations from a (hydrostatic) equation of state. The origin of the deformations is traced to shear stresses transmitted to the sample through the pressure medium.

    Notes: Zusammenfassung Der 44-jährige Patient stellte sich zur Abklärung von Angina pectoris, Dyspnoe bei leichter Belastung und wiederholten Synkopen vor. Die Symptomatik bestand bereits seit der Kindheit und war in letzter Zeit deutlich progredient gewesen. Es zeigten sich deutliche Zeichen chronischer Hypoxie wie Trommelschlegelfinger, Uhrglasnägel und Polyglobulie sowie eine respiratorische Partialinsuffizienz mit Zeichen der Hyperventilation in der arteriellen Blutgasanalyse. In der Kontrastmittelechokardiographie, Sauerstofftestatmung und Lungenszintigraphie ergab sich der Verdacht auf eine pulmonale arteriovenöse Fistel mit Rechts-Links-Shunt, die pulmonalangiographisch als solitäre Fistel zwischen Mittellappenarterie und rechter oberer Lungenvene bestätigt werden konnte. In einem zweizeitigen komplikationslosen Kathetereingriff erfolgte der Fistelverschlußüber Metallspiralen. Im bisherigen Nachbeobachtungszeitraum von vier Monaten ist der Patient beschwerdefrei.

    Notes: Abstract Angle-dependent dip coating (ADDC) is a modified dip coating technique that offers advantages for the production of optical interference filters. In contrast to conventional dip coating (DC), the substrate is withdrawn from the coating solution under an angle of inclination. Thereby, the two surfaces of the substrate are coated with individual film thicknesses. An experimental setup for ADDC has been built and the decisive process influences on coating thicknesses have been evaluated. In order to gain full control over the individual layer thicknesses, reaching from 20 nm to 160 nm, it is necessary to vary the following process parameters: lifting speed, angle of inclination and concentration of the dipping solution. The results of coating experiments prove the advantages of ADDC over DC. A first example aims at reducing the number of coating steps: an ADDC long pass filter produced in 10 coating steps reaches the same optical performance as a conventional DC filter made in 16 steps. A second example demonstrates the possibility to improve quality: a commercial DC beam splitter can be improved with respect to the flatness of transmission and reflection curves when being produced in 4 steps by ADDC instead of 8 steps by DC. Furthermore, ADDC offers the possibility to fabricate even narrow band pass filters, which are naturally difficult to obtain by conventional DC.
