    Notes: Abstract. Lead, cadmium, mercury, chromium, selenium, and manganese concentrations were measured in the breast feathers of 25 pairs of Franklin's Gulls (Larus pipixcan) and in their eggs from a breeding colony at Agassiz National Wildlife Refuge in Northwestern Minnesota. Metal concentrations in eggs represent metals sequestered in the egg by females at the time of egg formation; while metal concentrations in parents represent concentrations of metals in the blood supply at the time of feather formation. There were no significant sexual differences in metal concentrations in feathers, assuming the male to be larger of each pair, but there were significant differences between the concentrations of metals in parents and their eggs. Eggs had significantly higher concentrations of selenium and chromium, but significantly lower concentrations of all other metals than the feathers of their parents. There were few significant correlations among metal concentrations within the egg or within the feather of females, but there were correlations for the feathers of males. Lead and cadmium in feathers were positively correlated for both males and females. Chromium concentrations in eggs were generally higher than reported in the literature. The concentrations in eggs and the feathers of females were positively correlated for mercury, and negatively correlated for chromium and manganese.

    Description / Table of Contents: Summary Natural and artificial UV radiation are environmental factors with both beneficial and harmful biological effects. This article will explain the physical measurement quantities and their relation to the biologically effective dose and will summarize the present technical state of the art of personal UV monitoring. In practical use are dosimeters based on polysulphone, a polymer which undergoes changes in its optical properties upon irradiation with UV. Other systems determine the UV dose by quantifying damage induced in Bacillus subtilis spores upon UV exposure. An electronic UV sensor represents a new and interesting development. Personal UV dosimeters will become an useful tool in both clinical and scientific areas within dermatology.
    Notes: Zusammenfassung Natürliche und künstliche UV-Strahlung gehört zu den Umweltfaktoren, die sowohl positive als auch schädigende Wirkungen auf den Organismus haben. Neben den physikalischen Meßgrößen und ihrem Zusammenhang zur biologisch wirksamen Dosis wird der aktuelle technische Stand der UV-Personendosimetrie dargestellt. Derzeit kommen Dosimeter auf der Basis von Polysulfon zum Einsatz; Polysulfon ist ein Polymer, das seine optischen Eigenschaften unter UV-Exposition ändert. Ferner sind Systeme verfügbar, bei denen die Schädigung von Bacillus subtilis Sporen unter UV-Exposition zur Dosisbestimmung verwendet wird. Als aktuelle Entwicklung ist ein elektronischer UV-Sensor mit nachgeschaltetem Datenspeicher besonders interessant. Diese UV-Personendosimeter sind für unterschiedliche Anwendungen in der Dermatologie von Bedeutung.

    Description / Table of Contents: Abstract. Herpes gestationis (HG) is a rare autoimmune disease of pregnancy. We report on five patients in whom the clinical diagnosis was confirmed by histology, immunofluorescence and immunoblot studies. The time of onset ranged from the 8th week of pregnancy to the 1st day post partum. In one patient erosions and ulcerations of the oral cavity predominated while the remainder of the skin was nearly unaffected. One patient had intense pruritus without any skin eruptions during the final 8 weeks of her pregnancy, developing bullous lesions on the 1st day post partum. The other patients initially suffered from pruritic polymorphic skin lesions, followed by vesiculae and bullae. In the serum of all patients, the complement-fixing HG factor was found. Immunoblot studies with epidermal and dermal extracts demonstrated the binding of the antibodies to a 180-kD protein of the epidermis. All patients improved markedly with medium dosages of systemic corticosteroids. In four cases the newborns were healthy and mature, and in one case, the infant met all criteria for a "small-for-dates" baby. We discuss both clinical spectrum and diagnostic options of HG and review new aspects related to the pathogenesis of this disease.
    Notes: Zusammenfassung. Der Herpes gestationis (HG) ist eine seltene Autoimmundermatose der Schwangerschaft. Wir berichten über 5 Patientinnen, bei denen die klinische Diagnose histologisch, immunfluoreszenzoptisch und mittels Westernblot bestätigt wurde. Der Beginn der Erkrankung lag zwischen der 8. Schwangerschaftswoche und dem 1. Tag postpartal. Bei einer Patientin dominierten ausgeprägte Erosionen und Ulzera der Mundschleimhaut, während die übrige Haut auch im weiteren Krankheitsverlauf kaum betroffen war. Bei den anderen Fällen waren starker Pruritus und polymorphe Hautveränderungen die initialen Symptome. Bei einer Patientin bestand seit der 31. Schwangerschaftswoche (SSW) starker Pruritus ohne Hautveränderungen, und es kam erst ab dem 1. Tag postpartal zum Auftreten infiltrierter Plaques und Blasen. Mittlere systemische Kortikosteroiddosen reichten bei allen Patientinnen zur weitgehenden Unterdrückung der Symptomatik aus. In 4 der 5 Fälle wurden regelrecht entwickelte Kinder entbunden, eines der Kinder zeigte hingegen die Kriterien eines "small for date baby". Bei allen Patientinnen gelang die Diagnose serologisch durch Nachweis des Herpes-gestationis-Faktors in der indirekten Immunfluoreszenz, während der konventionelle Nachweis von IgG-Serumantikörpern negativ war. Im Immunoblot epidermaler Extrakte präzipitierten die Seren von 4 Patientinnen ein 180-kD-Protein. Anhand dieser Fälle diskutieren wir das klinische Spektrum und die diagnostischen Möglichkeiten beim HG und geben einen Überblick über neue Aspekte zur Pathogenese dieser Erkrankung.

    Notes: Abstract We have analysed 1173 cystic fibrosis (CF) chromosomes from Switzerland for eight mutations in the CF transmembrane conductance regulator (CFTR) gene. This permitted the identification of 88.5% of all mutations present. A novel insertion mutation in exon 20 of the CFTR gene, 3905insT, was discovered. This mutation accounted for 4.8% of CFTR gene mutations in Switzerland and has since been identified in other populations of probable Swiss descent. It is associated with a highly variable clinical phenotype but always with pancreatic insufficiency. Haplotype analysis with three intragenic microsatellites in the CFTR gene showed that the mutation is associated with a haplotype rarely identified on other CFTR alleles and, therefore, that the frequency of the mutation in Switzerland is explained by a founder effect of a relatively recent mutation event.

    Notes: Abstract Objectives: In a field study we evaluated the workplace pollution occurring during conscious sedation with sevoflurane in adults. Methods: Sevoflurane was given in 100% oxygen at a fresh gas flow rate of 3 l/min via a nasal mask. This was conducted in 25 patients scheduled for surgical procedures performed under regional anesthesia. Trace concentrations of sevoflurane were directly measured every minute in the breathing zone by means of a photoacoustic infrared spectrometer in an operating room with an air turnover of 20 changes/h. Results: The mean sedation time was 49.6 ± 20.4 min. The average vaporizer setting of the anesthesia machine was 1.63 ± 0.6 vol%, resulting in a patient's mean end-tidal sevoflurane concentration of 0.78 ± 0.2 vol%. The 8-h time-weighted average was calculated to be 0.58 ppm sevoflurane. Conclusions: The trace gas concentrations were low and comparable with values obtained under inhalation induction in adults and children. Although no occupational standard for sevoflurane is currently defined, the measured values are clearly under the standards recommended for enflurane (20 ppm) and isoflurane (10 ppm) by the European health authorities. We conclude that the new anesthesiologic method of conscious sedation with sevoflurane in adults using a nasal mask would not result in a violation of occupational standards, provided that the future value set for sevoflurane would be similar to those recommended for isoflurane or enflurane.
