    Notes: Summary The antineoplastic activity of 4-chloro-2-(2,6-dichloro-4-hydroxyphenyl)-1-ethyl-6-hydroxyindole (D 15413) was determined in several estrogen-dependent mammary tumor models. The growth of DMBA-induced rat mammary carcinomas was inhibited by doses ranging from 2 to 18 mg/kg. A dose of 6×12 mg/kg per week p.o. reduced the tumor area by 82% (control+192%). D 15413 was also active against MNU-induced rat mammary tumors and transplanted MXT tumors of the mouse. In vitro, the growth of estrogen receptor positive MCF-7 breast cancer cells was inhibited by D 15413 (10−8–10−5 M). A mode of action considering endocrine and cytotoxic effects is discussed.

    Notes: Summary The ability of active specific immunotherapy to enhance immune responses to autologous tumor-associated antigens (TAA) and to prolong the disease-free interval was evaluated in patients with Dukes B2 and C colorectal carcinoma who had undergone potentially curative resections. Patients were sensitized in the early postoperative period with irradiated autologous adenocarcinoma cells mixed with bacillus Calmette-Guérin (BCG) to yield either a low-dose vaccine (3×106 tumor cells) or a high-dose vaccine (1×107 tumor cells). Six of seven patients who received the low-dose vaccine developed delayed-type hypersensitivity (DTH) responses to autologous tumor cells upon completion of the vaccination, whereas all four patients receiving high-dose vaccine displayed a positive DTH response. However, DTH responses to autologous TAA waned within 3 months in all patients receiving the low-dose vaccine; DTH responses persisted for 3 months in three of the four high-dose vaccine patients. In vitro lymphoproliferative responses to TAA correlated with DTH responses to autologous tumor cells. Active specific immunotherapy appeared to induce specific immune responses either in vitro or in vivo to autologous TAA because it did not induce responses to autologous mucosa cells. There were no complications caused by BCG or tumor cells. This series demonstrates that active specific immunotherapy is a nontoxic treatment that augments immunity to autologous TAA.

    Notes: Summary We examined opioid binding in fractions with disconnected nerve endings (secretosomes) which were prepared from porcine neurohypophyses by centrifugation in a discontinuous Percoll gradient. Specific (= displaceable) binding was observed with 3H-etorphine and with 3H-diprenorphine, two ligands with low selectivity for distinct opiate receptor sub-classes. No displaceable binding was found with the prototypic mu- and delta-ligands 3H-dihydromorphine and 3H-(D-Ala, D-Leu) enkephalin. Displacement of 3H-diprenorphine binding was almost absent with the selective mu- and delta-ligands morphiceptin and ICI-174864. Partial displacement occurred with the selective kappa-ligand U-50488 and with dynorphin (1–8). Binding of 3H-etorphine was stereo-specific. 3H-diprenorphine binding was saturable with a KD between 2 and 4 nM. Maximum of opiate binding activity was detected in the fractions with accumulated secretosomes. By autoradiography specific 3H-diprenorphine binding is shown to be mainly associated with secretosomes. In imunocytochemical preparations an oxytocin antibody was immunoreactive in 85% of the secretosomes in the fraction with highest opiate binding. These fractions in radioimmunoassays exhibited the largest contents in oxytocin and low vasopressin levels. The data therefore suggest that in the porcine neurohypophysis opioid binding sites of the kappa-type occur in secretory endings presumably of the oxytocin type.

    Notes: Summary Antiestrogens and partial estrogens of the stilbene (1 and 4), triphenylbutene (2) and diphenylethane (3) series were tested for their potential prostatic tomor inhibiting activity. Compounds 1 and 2 exerted a strong inhibitory activity on prostate and seminal vesicle weight of intact rats and mice, whereas the strong antiestrogen 3 and compound 4 had no or only a slight effect. The tumor inhibiting activity of 1 and 2 on the hormone-dependent R 3327 Dunning prostatic carcinoma of the rat was strong and comparable to that of castration or administration of the potent estrogen DES. Compounds 1-4 had no direct antiandrogenic effect in castrated, testosterone-substituted rats and mice, and no affinity for the androgen or progesterone receptor. To the estrogen receptor from prostatic tumor cytosol, however, 1-4 had good receptor affinities. As the partial antiestrogen 1 and the partial estrogen 2 have much lower estrogenic properties than DES, but still have strong prostatic tumor inhibiting properties, they may offer a suitable alternative to conventional therapy of prostate carcinoma because of their possibly low estrogenic side effects.

    Notes: Summmary Both hydroxy groups of 1,1-bis-(4-hydroxyphenyl)-2-phenyl-but-1-ene (1) were esterified with the β-chloropropionate (2), β-bromopropionate (3) and acrylate functions (4). Linkage of these cytotoxic moieties reduced the estrogen receptor affinities of these compounds, especially in the case of the β-haloesters only slightly compared with the affinity of the unsubstituted carrier molecule 1. The estrogenic potencies of the derivatives 2–4 and of 1 were nearly identical. The alkylating activities increased in the order 2, 3, and 4. Because of their alkylating activities, all cytotoxic esters showed an irreversible mode of binding to the estrogen receptor. In vitro, 3 in particular, exerted better growth inhibition of the hormone-dependent MCF-7 cell line than of the hormone-independent MDA cell line. In vivo, all cytotoxic derivatives caused almost complete inhibition of the growth of the hormone-dependent MXT M3.2 mouse mammary tumor, whereas growth of the hormone-independent MXT-Ovex tumor was much less inhibited. In all tumor models, the antitumor effect of the cytotoxic esters was better than that of the unsubstituted carrier. Therefore, the antitumor activity of these esters could be due not only to their improved pharmacokinetic properties compared to 1, but also to a selective estrogen receptor-mediated cytotoxic action.

    Description / Table of Contents: Résumé Le traitement des traumatismes chez l'enfant s'est modifié et amélioré de multiples façons. En premier lieu, grâce à la planification par les collectivités communales, régionales et nationales de l'organisation de la chirurgie pédiatrique en ce qui concerne l'accueil, les soins d'urgence, les centres chirurgicaux et les centres de soins postopératoire où sont traités les enfants victimes de traumatismes. La communication par voie téléphonique par radio et/ou centre informatique reliant tous les maillons de la chaîne de soins permet de traiter les blessés de façon rationnelle, cette amélioration allant de pair avec la formation adéquate des membres du corps chirurgical appelés à leur donner des soins adaptés aux techniques et aux lésions particulières de la chirurgie infantile. Les nouvelles méthodes d'imagerie médicale qui permettent de déceler avec précision les lésions intra-craniennes ou des autres cavités du corps ont modifié radicalement les conditions du diagnostic et du traitement précoce des traumatismes de l'enfant. Elles ont permis par exemple le traitement conservateur des lésions de la rate, du foie et du cerveau. Ces progrès se sont développés parallèlement à ceux réalisés dans le traitement de l'infection et dans la nutrition du blessé. Beaucoup a été fait, cependant le traumatisme reste la cause de mort la plus fréquente de l'enfant âgé de 1 à 14 ans. C'est la prévention qui représente la réelle solution du problème.
    Abstract: Resumen El manejo de víctimas de trauma en la edad pediátrica ha evolucionado y mejorado en numerosos aspectos. En primer lugar, las unidades asistenciales, salas de urgencias, centros médicos y facilidades terciarias de atención pediátrica han sido reunidas por los planes comunitarios, regionales y nacionales, los cuales han tenido en cuenta las necesidades de las poblaciones y los recursos de cada institución. La comunicación telefónica y los transmisores de radio y de informatión computadorizada han integrado a una serie de facilidades médicas que ahora pueden garantizar que un niño será tratado en forma adecuada o será transferido a otra unidad del sistema. La educatión de todos los miembros de los servicios médicos de emergencia en técnicas especiales, en el manejo de equipos y en la atención de las necesidades de los pacientes pediátricos ha sido esencial para el manejo adecuado de los niños lesionados. El adelanto en la imagenología corporal, que permite obtener detalles de lesiones intracraneanas y de otras cavidades, ha modificado en forma radical el diagnóstico y el manejo clínico inicial de los pacientes lesionados. Esto, a su vez, ha hecho posible la toma de decisiones más precisas, tales como la preservatión esplénica y el manejo orientado a la preservación de órganos tales como el hígado y el cerebro. Todos estos avances han ocurrido en un momento durante el cual el tratamiento de la infección y la respuesta a los requerimientos nutricionales han exhibido progresos dramáticos. Mucho ha sido logrado, pero los accidentes todavía matan a más niños en las edades de 1 a 4 años, que todo el resto de causas combinadas. Por lo tanto, la preventión significa la solución real para un continuado progreso en el futuro.
    Notes: Abstract The handling of trauma victims in the pediatric age group has undergone change and improvement in a number of ways. First, receiving facilities, emergency rooms, medical centers, and tertiary pediatric care facilities have all been brought together by community, state, regional, and national planning which has taken into account population needs and the resources of each facility. Communication by telephone, radio, and computerized information transmitters has linked together a series of medical facilities which now assures that each child is treated appropraitely or passed along to a different unit in the chain. The education of all members of the emergency medical services to the special techniques, equipment, and needs of pediatric patients has been essential to proper handling of injured children. Improvement in body imaging, which allows details of injured sites intracranially and in other body cavities, has changed radically the diagnosis and early clinical management of all injured patients. This, in turn, has allowed more precise surgical decisions such as splenic salvage, and conservative handling of such organs as the liver and brain. All of these advances have taken place on a stage and in a time in which the handling of infection and the response to nutritional needs have improved so dramatically. Much has been done, but accidents still kill more children ages 1 to 14 years than all other causes combined. Thus, prevention holds the real solution for future progress.

    Notes: Abstract Thyrotropin releasing hormone (TRH) was administered intravenously to ten patients with Alzheimer's Disease (AD) in a high-dose paradigm, thought to maximize central nervous system effects and potentially produce facilitation of cholinergic function, a known property of the neuropeptide. Acute effects of TRH on behavioral, cognitive and physiologic measures were assessed after patients received 0.1 mg/kg TRH, 0.3 mg/kg TRH and placebo, the higher TRH dose and placebo being given in a randomized, double-blind fashion. Patients showed statistically significant increases in arousal and improvement in affect, as well as a modest improvement in semantic memory, all after receiving the higher TRH dose. Both TRH doses produced transient rises in systolic blood pressure, with no effect on diastolic blood pressure, heart rate or temperature. This study suggests that high-dose TRH can be safely administered to AD patients and is neurobehaviorally active; further studies are needed to determine the extent and mechanism of the cognitive and psychobiological properties of this peptide in AD and other neuropsychiatric disorders.

    Notes: Summary Light-induced degradation of photoreceptor membrane in the crayfish was studied by quantitative light and electron microscopy. The production of lysosomal organelles within the photoreceptor cells was enhanced by presenting the light stimulus intermittently (i.e., flicker) or by doubling its intensity. The enhancement was seen primarily as an increase in the number and size of multivesicular bodies. As these stimulus conditions are likely to facilitate intracellular Ca++ fluxes, the results are compatibl with recent speculations that Ca++ ions may regulate membrane degradation. To test the possibility that Ca++ acts as a signal coupling receptor stimulation with membrane loss, retinas were incubated in the dark with the ionophore A23187 in the presence or absence of external Ca++. The results demonstrate that A23187 produces a Ca++-dependent increase in lysosomal organelles, predominantly multivesicular bodies. These data are consistent with a role for intracellular Ca++ in the degradative process; however, the exact locus of the effect is unclear.
