    Notes: Summary A hyaluronan binding protein (HABP), extracted from cartilage, was biotin-labelled and used for histochemical localization of hyaluronan (HA) in tissue sections. Various tissues were fixed for a mixture of formaldehyde and glutaraldehyde during microwave irradiation. The microwave oven when set at 700 W and 45°C yielded an intense and specific staining of HA. Under these conditions the relative proportion of the two aldehydes did not influence the staining intensity. Aldehyde fixation during microwave irradiation for HA histochemistry, (1) saves time, (2) eliminates the use of cetylpyridinium chloride (CPC), and (3) improves the reproducibility.

    Notes: Commercially pure titanium implants were inserted in rabbit tibia for 3, 6 and 12 weeks. Each rabbit had two implants inserted, one for removal torque measurements and the other for histomorphometrical analysis. Light microscopic observations revealed that there was a continuing bone remodelling, with new bone formation in the periosteal region after 3 and 6 weeks, which diminished with time, i.e. up to 12 weeks of follow-up. A higher removal torque was observed with increasing time of implant insertion. The removal torque values for the 12 week samples were converted to three different shear forces depending on three different theoretically calculated implant to bone attachment levels. The mean shear forces related to the entire length of the implant surface was 0.6 N mm-2. If considering the length of the implant inside the cortical bone only, the mean shear force was 1.9 N mm-2, and if the bone-metal contact length as related to an estimate of the bone-implant contact a mean shear force of 14.8 N mm-2 was calculated. Histomorphometrical measurements revealed more bone-metal contact as well as a larger bone area in the threads with increasing time of insertion. Statistically significant differences were observed between all measurements of the 3 and 6 week samples and between the 3 and 12 week samples. For the 6 and 12 week sections a statistically eignificant difference could be demonstrated only when comparing bone areas in the three best consecutive threads located in the cortical region.

    Notes: Abstract Commercially pure (c.p.) titanium and Ti-6Al-4V implants were inserted in the abdominal wall of rats. The surrounding fluid space, inflammatory cells and fibrous capsule were evaluated after 1, 6 and 12 weeks. Light-microscopic morphometry demonstrated a fluid space around both implant materials which gradually decreased with time. Macrophages were preferentially distributed close to the implant surface in the innermost zone (0–25 µm from the surface). In contrast, fibroblasts and endothelial cells were located mainly in the outer three zones (25–100 µm from the surface). At all time periods studied and around both materials, lymphocytes were detected throughout the surrounding tissue. The outer border of the fibrous capsule, which consisted of macrophages, fibroblasts, endothelial cells and collagen, was difficult to define, in particular during the early phase of healing. At later time stages, 6 and 12 weeks, no difference in width (60–90 µm) was observed between the two materials. No major quantitative differences with respect to the number of different cells, fluid space width and fibrous capsule thickness were noted between the two materials studied. The observed mild inflammatory reaction and the absence of statistically significant differences between c.p. titanium and Ti-6Al-4V in soft tissue indicate that both materials could be suitable for use in soft tissues. In the context of previous comparative studies it may be concluded that the animal species as well as the different implantation locals play an important role in the determination of biocompatibility.

    Notes: Nitrogen-ion implantation is a surface modification of interest for biomedical materials. In this study screw-shaped commercially pure titanium and Ti6AI4V alloy implants were nitrogen-ion implanted and analysed by scanning electron microscopy (SEM) and scanning Auger electron spectroscopy (AES). The surface topography was essentially similar for treated and non-treated samples. AES survey spectra showed an incorporation of nitrogen into the surface of the nitrogen-ion treated commercially pure titanium and Ti6AI4V implants, as judged by the ratio between the intensities of the peaks at 390 and 420 eV. AES depth profiles revealed a similar oxide thickness (7.5 nm) for the nitrogen-ion implanted and the non-treated samples. In nitrogen-ion implanted screws nitrogen was detected up to depths of about 150 nm below the surface, with a maximum nitrogen concentration at about 50 nm. Light microscopic examination of the 10 μm-thick ground sections 3 months after the insertion of nitrogen-ion implanted and non-treated commercially pure titanium and Ti6AI4V screws in the proximal tibial methaphysis of rabbits showed that bone filled a large portion of the area within the threads. A fibrous capsule was not observed. Light microscopic morphometry did not reveal any statistically significant differences in bone-metal contact or bone area within the threads of nitrogen-ion treated and non-treated implants. The “mirror-image” analysis showed that for all implants examined significantly more bone was present immediately outside than inside the threads. The results from this study indicate that nitrogen-ion implanted, screw-shaped, commercially pure titanium and Ti6AI4V implants heal as well in cortical bone as non-treated samples.

    Notes: Screw-shaped implants with an outer diameter of 3.7 mm and a total length of 8 mm were made from rods of commercially pure titanium (c.p. Ti) and commercially pure zirconium (c.p. Zr). Prior to insertion in rabbit tibia for 12 weeks, the surface roughness of two randomly chosen c.p. Ti and two c.p. Zr machined implants of the batch were numerically and visually described with a TopScan 3-D system. Irrespective of implant material, they demonstrated rather similar surface roughnesses, for example, the R a and R q values (that describe the average deviation from the mean line) were 0.79 and 1.18 for c.p. Ti versus 0.56 and 0.86 for c.p. Zr. Each rabbit had four implants inserted; two made of c.p. Ti in one leg and two made of c.p. Zr in the other. The bone tissue reactions to the materials were qualitatively and quantitatively examined. Removal torque tests were performed on the distal implants and histomorphometrical evaluations of the proximal ones. No qualitative or quantitative bone differences were observed. Removal torque measurements demonstrated a mean of 26.2 N cm±9.6 for c.p. Ti versus a mean of 25.9 N cm±7.1 for the c.p.Zr implants. Histomorphometrical comparisons of the bone-to-metal contact revealed a mean of 29 ±10.2% for c.p. Ti and a mean of 19±5.5% for the c.p. Zr samples. The mean bone area in the threads around the c.p. Ti samples was 49±16.6% compared to 43±7.9% for the c.p. Zr group. Commercially pure titanium and zirconium implants seemed to be well accepted by the bone bed after 12 weeks of insertion in rabbit bone.

    Notes: Summary This study aimed to use a classical model to analyse whether alterations in performance due to shortterm changes in diet are reflected in peripheral basal anabolic and catabolic hormone concentrations. Six healthy students (two women and four men) performed a cross-over study including three test situations. The reference test was preceded by each subject's normal unrestricted diet of western type (N). After this, three of the subject's were provided a carbohydrate-rich diet (CHO) for 3 days followed by the second test. Following this test, a carbohydrate deficient, protein/fat rich diet (F) was provided for another 3 days followed by the third test. The remaining three subjects had these diets in reverse order. Basal concentrations of cortisol (C), sex hormone-binding globulin (SHBG), insulin-like growth factor (IGF-I), total-testosterone and non-SHBG-bound-testosterone (NST), the NST:C ratio and an ergometer cycle endurance test with measurements of submaximal as well as measurements of maximal oxygen uptake, heart rate, respiratory exchange ratio (R), free fatty acids (FFA), glycerol and lactate were investigated. All the subjects were involved in athletics for recreation and during the study they performed daily physical exercise according to their normal routine. After the F diet only two of the subjects completed the test. One woman dropped out after 16 min, two men dropped out after 14 min and one after 13 min of cycling. After the CHO diet all six subjects completed the ergometer cycle test. Considering the whole group, there were no significant differences in lactate or glycerol concentrations during the study. The FFA concentrations were significantly higher after consuming the F diet compared to the CHO diet; and correspondingly R was significantly increased after consuming the CHO diet compared to the F diet. These changes in physiological characteristics due to short-term dietary changes were not reflected in concomitant alterations in the concentrations of anabolic or catabolic hormones, or SHBG.

    Description / Table of Contents: Résumé Soixante-neuf patients présentant des entérocèles à la défécographie et 128 patients sands entérocèle mais avec d'autres anomalies ont été investigués afin d'identifier les troubles fonctionnels colo-rectaux concomittants et leur relation avec l'entérocèle. De 69 patients avec entérocèle, 38 (55%) présentent simultanément une intussusception et 26 (38%) un prolapsus du rectum. Des anomalies physiologiques lors de manométries et d'investigations électrophysiologiques sont plus fréquentes chez des patients porteurs d'une entérocèle. Une hystérectomie préalable augmente le risque de la formation d'une entérocèle. Cette étude démontre que les patients porteurs d'une entérocèle doivent faire l'objet d'investigations minutieuses pour exclure toute autre lésion avant d'envisager un traitement. Des investigations ultérieures devront être envisagées pour démontrer le rôle de l'entérocèle chez des patients souffrant de troubles de l'exonération.
    Notes: Abstract Sixty-nine patients with enterocele on defaecography, and 128 patients without enterocele but with other abnormal findings were investigated to identify concomitant colorectal disorders and their relationship with enterocele. Of the 69 patients with enterocele, 38 (55%) had concomitant rectal intususception and 26 (38%) rectal prolapse. Abnormal physiological findings on anorectal manometry and electrophysiology were more common in patients with enterocele. Previous hysterectomy increased the risk of enterocele formation. The study has demonstrated that patients with enterocele should be investigated thoroughly for other lesions before treatment is planned. Further investigation of the role of enterocele in patients with defaecation disorders is required.

    Description / Table of Contents: Résumé Parmi 71 patients présentant une réaction paradoxale du sphincter anal, 54 présentaient une constipation ou une dyschésie et 17 étaient incontinents. Ces patients avaient eu une défécographie, un temps de transit colique, une manométrie ano-rectale et une électromyographie. 96% d'entre eux présentaient des perturbations additionnelles de l'anatomie et de la physiologie ano-rectales. 70% de ces patients avaient une défécographie anormale et 42% un temps de transit colique prolongé. Une diminution de la pression anale maximale et de la pression de retenue maximale, indiquant une perturbation de la fonction des sphincters anaux, pourraient être une raison de l'incontinence de ces patients avec réaction paradoxale du sphincter. Cette réaction paradoxale survenait dans le muscle pubo-rectal et dans trois sites examinés du sphincter externe. Il est donc suffisant d'enregistrer l'électromyogramme dans un seul muscle et en un seul point pour diagnostiquer une réaction paradoxale du sphincter. L'absence d'un réflexe normal de fermeture en électromyographie témoigne d'une réaction paradoxale du sphincter. La dénervation était plus prononcée au niveau du sphincter externe que dans le muscle pubo-rectal. Le nerf honteux interne droit était plus souvent atteint que le nerf gauche.
    Notes: Abstract Of 71 patients with paradoxical sphincter reaction, 54 had symptoms of constipation or outlet obstruction and 17 were incontinent. The patients were investigated with defecography, colon transit time, anorectal manometry and electromyography. Ninety-six percent of the patients had additional changes in anorectal anatomy and physiology; 70% of the patients had abnormal defecography and 42% had delayed colon transit time. Decreased maximal anal pressure (MAP) and maximal squeeze pressure (MSP), indicating impaired function of the anal sphincters might be one reason for incontinence in patients with paradoxical sphincter reaction. The paradoxical reaction occurred in the puborectalis muscle and in three tested sites in the external sphincter. It is sufficient to record the EMG activity in one muscle and at one point to diagnose a paradoxical sphincter reaction. The absence of a normal closing reflex on electromyography is evidence for a paradoxical sphincter reaction. Denervation was more pronounced in the external sphincter than in the puborectalis muscle. The right pudendal nerve was subjected to damage more often than the left nerve.
