    Abstract: BACKGROUND: No studies have so far evaluated the impact of the metabolic syndrome (MetS) as an entity on ovarian cancer risk. The authors aimed to examine the association between factors in the MetS, individually and combined, and risk of ovarian cancer incidence and mortality. METHODS: Altogether, 290,000 women from Austria, Norway and Sweden were enrolled during 1974-2005, with measurements taken of height, weight, blood pressure and levels of glucose, cholesterol and triglycerides. Relative risks (RRs) of ovarian cancer were estimated using Cox regression for each MetS factor in quintiles and for standardized levels (z-scores), and for a composite z-score for the MetS. RRs were corrected for random error in measurements. RESULTS: During follow-up, 644 epithelial ovarian cancers and 388 deaths from ovarian cancer were identified. There was no overall association between MetS and ovarian cancer risk. Increasing levels of cholesterol [RR 1.52, 95% confidence interval (95% CI) 1.01-2.29, per 1-U increment of z-score] and blood pressure (RR 1.79, 95% CI 1.12-2.86) conferred, however, increased risks of mucinous and endometrioid tumours, respectively. In women below the age of 50 years, there was increased risk of ovarian cancer mortality for MetS (RR 1.52, 95% CI 1.00-2.30). Increasing levels of BMI (RR 1.17, 95% CI 1.01-1.37) conferred increased risk of ovarian cancer mortality in women above the age of 50 years. CONCLUSION: There was no overall association between MetS and ovarian cancer risk. However, increasing levels of cholesterol and blood pressure increased the risks of mucinous and endometrioid tumours, respectively. Increasing levels of BMI conferred an increased risk of ovarian cancer mortality in women above the age of 50 years.

    Notes: Summary Fifty-one consecutive patients with metastatic lesions of the cervical spine were treated surgically. The most common primary tumor types were breast cancer and myeloma. In 14 (27%) patients, the cervical lesion was the first manifestation of the malignancy. All patients suffered from severe pain but only six had long tract symptoms. Five tetraparetic patients were confined to bed. Vertebral body collapse occurred in 73% of cases. The surgical technique was individualized according to the patient's general condition, the site of metastasis on the vertebra, and the level and number of levels bearing in mind that the treatment is palliative in nature. The goal of treatment was a better quality of life. In the upper cervical spine the technique described by Sjöström et al. was used, if technically possible. If the odontoid process had been totally destroyed, an occipitocervical stabilization was chosen. In the lower cervical spine, an anterior approach was used to resect the tumor growth. Anterior support was provided with bone cement if the patient was not expected to survive long; otherwise bone grafting was used. In cases with two or more levels of involvement, a combined anteroposterior stabilization was usually performed. Good pain relief was achieved postoperatively. The operation was generally well-tolerated by the patients, mild dysphagia being the most common complaint. One patient died 2 days postoperatively of heart failure, giving a post-operative mortality of 2%. Rhizopathy symptoms were relieved totally in 15 patients and partially in 6. Five of six tetraparetic patients could walk postoperatively. Mechanical failures occurred three times, due to local progress of the metastatic lesion, which led to reoperation on two occasions. The average postoperative survival was 12 months. Patients with metastases from breast cancer or myeloma had the longest survival. Our study suggests that stabilization is important in the cervical spine when metastatic lesions jeopardize stability. Stabilization is effective in relieving pain and preventing paraplegia.

    Notes: Abstract. We present the first polynomial time algorithm that finds the shortest route in a simple polygon such that all points of the polygon are visible from the route. This route is called the shortest watchman route, and we do not assume any restrictions on the route or on the simple polygon. Our algorithm runs in worst case O(n 6 ) time, but it is adaptive, making it run faster on polygons with a simple structure.

    Notes: Summary Forty consecutive patients with 19 fractures and 21 fracture-dislocations in the lower cervical spine were treated prospectively with open reduction and interbody fusion, using the AO titanium locking screwplate system. There were 30 men and 10 women with a mean age of 36 years (range 16–90 years). Eight were admitted tetraplegic, 12 tetraparetic, and 6 had nerve root injuries. Eleven of the fractures and 18 of the fracture-dislocations were also plated posteriorly. Complications included two cardiac arrests, two transient neurological impairments, two severe gastrointestinal bleedings, and one esophageal fistula. At 2-year follow-up, three patients had died and one had emigrated. Three out of ten patients who had complete motor loss initially had regained useful muscle function, while incomplete motor loss usually returned to normal. All fusions had healed in a good or acceptable position. Twenty-four of the 60 posterior plates impinged on facet joints and five were loose. Six screws transgressed facet joints below the fusion. Ten posterior fusions extended to adjacent mobile segments by exuberant bony overgrowth. Ancillary posterior plating significantly reduced the range of neck mobility and also caused significantly more pain than anterior plate fixation alone.

    Notes: Abstract Using roentgen stereophotogrammetric analysis we recorded the three-dimensional movements in six knees with implanted Tricon-M prostheses and ten knees with Miller-Galante prostheses as the patients ascended a platform. Fourteen patients with normal knees were used as controls. The two prosthetic designs displayed decreased internal tibial rotation and the Tricon-M increased valgus rotation. A central point on the tibial articular surface had a more lateral position in the Tricon-M design and a more distal one in the Miller-Galante design compared to normal knees. Increased posterior displacement with increasing flexion was observed in both designs. When the normal knees were extended at full weightbearing the helical axes mainly shifted inclination in the frontal plane. In the prosthetic knees there was a tendency to anterior-posterior displacement of the axes as extension proceeded, especially in the Miller-Galante design. Translations along the helical axes were larger than normal in the Miller-Galante and smaller in the Tricon-M knees, reflecting differences in constraint of the two designs.

    Notes: Summary The enantioseparation of amphetamine, methamphetamine and various ring-substituted amphetamines by use of a chiral stationary phase carrying immobilized native β-cyclodextrin (β-CyD) selectors is reported. The system is evaluated for resolving the specified compounds directly without any derivatization and after derivatization with phenyl isothiocyanate (PITC), naphthyl isothiocyanate (NITC) and 6-aminoquinolyl-N-hydroxysuccinimidyl carbamate (AQC). This direct enantioseparation is compared with the features of indirect separation of diasteromeric derivatives after reaction with the optically pure Marfey's reagent employing a simple non-chiral alkyl-silica (RP-8) column. A selection of those methods best suited for each single amphetamine is given. Seventeen different samples of amphetamine, confiscated by the Swedisch police, were analyzed with respect to their enantiomeric composition. Within this set of samples synthesized by the same method no significant deviation from a racemic ratio could be observed.

    Notes: Experiments with an oscillatory vane in a wind tunnel reveal that the vane's oscillation frequency is directly proportional to the square root of the dynamic pressure of the flow. The linear relationship is observed for flow speeds up to the speed of sound in air, argon, and helium. The vane's oscillations are a measure of mass flow, or if used on an aircraft, of indicated air speed. In constant density media the oscillation frequency is directly proportional to flow speed, and a simple count of the oscillations is a measure of distance traveled. At given ambient temperature and pressure, the vane's frequency is the same at the speed of sound for all gases, irrespective of their molecular weight.
