    Notes: : The fetus is capable of producing catecholamines from early in gestation. Normal concentrations of these compounds are necessary for fetal and neonatal homeostasis, and for successfully completing the transition from fetal to neonatal life. Fetal and neonatal stress result in marked elevations of catecholamine levels, causing a large number of physiologic derangements. While fetal catecholamines are endogenously produced, maternal illness and anxiety can initiate the chain of maternal catecholamine production, causing dysfunctional labor and uterine hypoperfusion, and through this mechanism, fetal distress. Measures to reduce maternal anxiety should have a high priority in the management of labor. While nonpharmacologic methods may suffice, the use of analgesic agents in moderate doses may be protective to the fetus in selected circumstances.

Discussions with mothers should focus on their possible reluctance to gain or to accept normal fat deposits laid down during pregnancy, as well as how to deal with edema, nausea, indigestion and other discomforts. Changes in requirements for individual nutrients include those for protein, iron, folic acid, several vitamins, calcium, phosphorous and sodium. Discussions may include the potential teratogenic effects of food additives and contaminants, and, most importantly, the suspected effects of deficits and specific excesses. Concerns about excess relate in particular to vitamins A, D, and C, caffeine, alcohol and selected minerals.

Bearing-down efforts which lasted more than 5 seconds resulted in late decelerations of the fetal heart rate, marked falls in the maternal systolic and diastolic blood pressures and resultant delayed recovery of the fetal heart rate, with fetal hypoxia and acidosis in some cases. When women are not urged to bear down long and hard, their spontaneous efforts are usually within physiologic limits-5–6 seconds long-and fetal acidosis is thereby avoided.

    Notes: : Electronic fetal monitoring (EFM) of the fetal heart rate during labor and delivery has become a cornerstone of the new technology of obstetrics. Its routine use has been advocated, and the majority of labors in the U.S. are now monitored electronically. Recently, questions have been raised about the benefits of EFM. Review of the literature shows that only 4 randomized controlled clinical trials (RCTs) of EFM have been carried out. These show little if any benefit from the use of EFM in comparison to monitoring by stethoscope (fetoscope). The lack of diagnostic precision of EFM is reflected in high false positive and false negative rates of identification of fetuses in distress. Although many retrospective studies of EFM attempt to correlate use of EFM with declines in perinatal mortality in specific hospitals, the 4 RCTs showed no difference in mortality between babies monitored by auscultation and those who were electronically monitored. No differences have been found in morbidity, including neurologic status, between infants in the two groups. Electronic fetal monitoring has been associated with a significant increase in the cesarean section rate in most retrospective studies and all the RCTs.

    Notes: : The recent literature describing sibling presence at childbirth is reviewed. The parents of 43 children who were present at the births of their siblings were interviewed to assess the reactions of the children to the birth, the parents and the new baby. Based on this information, guidelines are presented to assist parents in preparing their children to be present at the birth, and to care for their older child during labor and delivery.

    Notes: Abstract We consider the problem of the existence of first order phase transitions in ferromagnetic spin systems at low temperatures. A criterion is given for the existence of phase transitions in terms of an algebraic system canonically associated with any interaction. The criterion involves finding out if the greatest common divisor of few polynomials belongs to the ideal generated by these polynomials. In connection with results published earlier, this work yields a description of all translation invariant (also of periodic and quasi-periodic) equilibrium states at low temperatures.

    Notes: Abstract The Positive Action conjecture requires that the action of any asymptotically Euclidean 4-dimensional Riemannian metric be positive, vanishing if and only if the space is flat. Because any Ricci flat, asymptotically Euclidean metric has zero action and is local extremum of the action which is a local minimum at flat space, the conjecture requires that there are no Ricci flat asymptotically Euclidean metrics other than flat space, which would establish that flat space is the only local minimum. We prove this for metrics onR 4 and a large class of more complicated topologies and for self-dual metrics. We show that ifR μ μ ≧0 there are no bound states of the Dirac equation and discuss the relevance to possible baryon non-conserving processes mediated by gravitational instantons. We conclude that these are forbidden in the lowest stationary phase approximation. We give a detailed discussion of instantons invariant under anSU(2) orSO(3) isometry group. We find all regular solutions, none of which is asymptotically Euclidean and all of which possess a further Killing vector. In an appendix we construct an approximate self-dual metric onK3 — the only simply connected compact manifold which admits a self-dual metric.

    Notes: Abstract A new criterion of the Cook-Kuroda type for the existence of the wave operator in the two-space scattering theory is introduced. The condition is quite simple, but it generalizes not only the original Cook-Kuroda condition but also its generalization recently given by Schechter. Specialized to the one-space case, it is actually equivalent to Schechter's condition for an optimal choice of factorization. An application to potential scattering leads to a new result.

    Notes: Abstract A generalization of graph theory is introduced and used to obtain Feynman parametric formulas relevant to renormalized amplitudes. The generalization of graph theory is based upon circuit coefficients instead of the usual incidence matrix. The parametric formulas presented are valid for amplitudes which have been renormalized, as in the Zimmermann formulation, by subtracting Taylor terms in momentum space.

    Notes: Abstract In a field theoretic framework we investigate generators of symmetry transformations induced by conserved local, not necessarily translationally covariant currents. Assuming the invariance of the vacuum and a mass gap, it is shown that the generator on one-particle states in general can be any polynomial of the generators of the Poincaré group and the internal symmetries. We give an example showing that the generator, defined as an integral over a conserved current, in spite of leaving the vacuum invariant, need not be self-adjoint.

    Notes: Abstract We introduce the surface tension for arbitrary spin systems and study its general properties. In particular we show that for a large class of systems, the surface tension is zero at high temperature. We also derive a geometrical condition for the surface tension to be zero at all temperature. For discrete spin systems this condition becomes a criterion to establish the existence of a phase transition associated with surface tension. This criterion is illustrated on several examples.

    Notes: Abstract Building upon Kostant's graded manifold theory, we present a new way of introducing spinors into the spacetime manifold, by expanding the algebra of functions on spacetime to a graded algebra. The elements of differential geometry are generalized to accomodate the expanded algebra of functions and in this enriched geometry we find the elements of supersymmetry and of supergravity theory. The geometrical role of the supergravity fields is discussed and a derivation of their transformation rules is given.
