    Notes: Summary Arterial oxygen and carbon dioxide tension was measured in spontaneously breathing rabbits whose intracranial pressure was raised by a freeze lesion, oil embolism, or hydrostatic pressure. Reports elsewhere had stated that high intracranial pressure led to hypoxaemia. In this series of experiments, arterial oxygen tension appeared to rise and carbon dioxide tension appeared to drop. Both changes were slight and not significant statistically. There was no evidence to suggest that arterial hypoxaemia is caused by high intracranial pressure.

    Notes: Summary Forty-three patients under the age of 19 in whom a lumbar disc protrusion was diagnosed and surgically demonstrated are reviewed. The symptomatology resembles that of adults. In 19 cases there was clear relation of trauma to the onset of symptoms. The results of surgical treatment are discussed.

    Notes: Summary Directly under the ependyma, and between the ependymal cells, there are basement membranes which form labyrinths connecting with the perivascular basement membranes of the subependymal vessels. The basement membranes exhibit differences in form, position, and distribution, and they can be distended by fluid absorption into large lacunae. The membranes can be identified as glycoprotein and glylcolipid substances. Electron microscopic studies have shown the differences between the basement membrane labyrinths of the human, the rabbit and the rat. In the human, the labyrinths contain isolated collagen fibrils. Basement membranes generally line all connective tissue spaces and form their interstitial borders. From this point of view, the subependymal basement membrane labyrinths can be seen as interstitial spaces near the ventricles, forming pathways between the ependyma and the subependymal capillaries or postcapillary veins.

    Notes: Summary Examination of the cerebrospinal fluid (CSF), and myelography, were performed in most of 89 consecutive patients with X-ray evidence of vertebral metastases to test the value of these examinations in the early detection of neoplastic involvement of the epidural space. Patients were divided into 3 groups. In 7 cases (group I), the vertebral metastases were asymptomatic. Fifty-nine (group II) had symptoms or signs of radicular lesions but not of spinal cord compression. Twenty-three showed various stages of spinal cord or cauda equina compression (group III). Involvement of the epidural space was demonstrated on myelography in almost 50% of patients from groups I and II by a partial (29%) or complete (19%) arrest of the opaque medium. In addition, five patients in group II with normal myelograms had increased levels of CSF protein. Leucocyte counts were within normal limits in all patients. Neoplastic cells were found in two cases. It is suggested that in patients with only radiological evidence of neoplastic involvement of the epidural space and complete myelographic blocks, laminectomy, followed by radiation therapy, should be performed. Patients showing partial blocks should receive radiation therapy only.

    Notes: Summary 12 patients, 10 women and 2 men, were operated on for removal of a jugular foramen neurinoma. Removal was total in 11 cases and operative mortality was 16.6%. As there are no clinical or instrumental signs that permit sure distinction between these tumours and those of the cerebellopontine angle, only a thorough radiological study of the jugular foramen allows correct diagnosis. Often, however, the diagnosis is made during the operation.

    Description / Table of Contents: Summary A comparison has been made of the relative merits of air, Conray 60, and Dimer X ventriculography in 100 cases. The great virtue of positive contrast ventriculography is its superior ability to demonstrate abnormalities in the midline and in the posterior cranial fossa. Tomoventriculography using a multiple layer cassette permits exact localization of intraventricular lesions without causing additional discomfort to the patient. The subtraction technique confers additional benefits. Positive contrast ventriculography has its limitations when there are large extraventricular masses.
    Notes: Zusammenfassung Es werden hinsichtlich Beurteilbarkeit und Nebenerscheinungen die Ergebnisse von je 100 Luft-, Conray 60- und DIMER-X-Ventrikulographien verglichen. Die Stärke der positiven Ventrikulographie liegt in der ungleich besseren Verträglichkeit und erheblich besseren Beurteilbarkeit bei Prozessen der Mittellinie und der hinteren Schädelgrube. Die Tomoventrikulographie mit mehrschichtiger Simultankassette erlaubt eine exakte Lokalisation intraventrikulärer Prozesse ohne weitere Belastung des Patienten. Ebenso ergibt das Subtraktionsverfahren einen besseren Überblick über Einzelheiten, ebenfalls ohne weitere Belastung des Patienten. Ihre Grenzen findet die positive Ventrikulographie bei Mißbildungen des Ventrikelsystems infolge Überlagerungseffekten und der notwendigen großen Kontrastmittelmengen.

    Notes: Summary In patients with severe head injuries ICP, MAP and CBF were measured continuously. In most patients there was a positive vasopressor response to increasing ICP, but the ICP/MAP ratio varied considerably in individual cases. CBF was diminished either by increasing ICP or by decreasing MAP. This effect was more marked with ICP above 40 mm Hg or MAP below 110 mm Hg. In terminal stages there was often a negative MAP/ICP ratio accompanied by massive cerebral hyperaemia.

    Description / Table of Contents: Summary Fifteen patients with intracranial aneurysms have been treated by coating the aneurysms with Biobond. Three patients died in the postoperative period. Only one patient had recurrent haemorrhage one year after operation. In this case the sac had not been wrapped completely at operation. Histological examination of this aneurysm revealed macrophage reaction and signs of slow disappearance of the plastic. Ten patients, who were followed up for a period of 3 to 8 years after operation, had no recurrent haemorrhage and were in good clinical condition, 8 of them doing full-time work. No complications caused by the material have been encountered. In 5 patients carotid angiography was performed after operation. In 2 cases the size of the aneurysm was diminished and in 3 cases was unchanged. There were no signs of thrombosis or narrowing of the adjacent arteries. We believe that Biobond is suitable for coating intracranial aneurysms that cannot be treated by clipping or ligature. As Biobond in vivo seems partially to disappear after long periods, it should be applied in thick layers.
    Notes: Zusammenfassung Bei 15 Patienten wurde eine Wandverstärkung von intrakraniellen Aneurysmen mit Biobond vorgenommen. 3 Patienten starben postoperativ. Eine erneute Aneurysmaruptur wurde nur bei einem Patienten beobachtet, bei dem die Aneurysmahinterwand aus operationstechnischen Gründen nicht mit dem Kunststoff beschichtet war. Die histologische Untersuchung dieses Aneurysmas ergab Hinweise auf einen Abbau von Biobond durch den Organismus. Bei überlebenden 10 Patienten waren in einem Nachuntersuchungszeitraum bis zu 8 Jahren keine Rezidivblutungen aufgetreten. 8 Patienten waren voll erwerbstätig. Nebenschädigungen durch Biobond wurden in keinem Fall beobachtet. Angiographien nach einem längeren Zeitraum nach der Operation bei 5 Patienten zeigten das Aneurysma bei 3 Patienten unverändert und bei 2 verkleinert. In keinem Fall traten Thrombosen oder Einengungen angrenzender Arterien ein. Biobond wird für die Behandlung von Aneurysmen für geeignet gehalten, welche sich nicht durch Clip oder Ligatur versorgen lassen. Im Hinblick auf den langsamen Abbau von Biobond durch den Organismus wird empfohlen, die Substanz in einer dickeren Schicht aufzutragen, als vom Hersteller empfohlen.
