    Description / Table of Contents: Abstract Although the treatment of fractures has repeatedly been documented throughout history, little attention has been paid to the historical development of the understanding of fracture healing and callus formation in itself. In antiquity, the capability of bone to regenerate was doubted, and Galen attributed the consolidation of a bone fracture to a sap (succus ossificus) that came from the food ingested. Later, in the eighteenth century, a haughty debate broke out as to the seat of the organ that controlled the by now accepted process of new bone formation. While Duhamel du Monceau (1700–1781) believed the periosteum was responsible for the callus formation – this idea being based on his observations of trees – Albrecht von Haller (1708–1777) and his scholars and disciples ascribed this function to the succus ossificus and to the bone marrow. The studies on experimental fracture healing that began during the seventeenth century resulted in the eighteenth-century discovery of the complex processes involved in the formation of callus with the aid of madder staining. The conciliation of the two major schools of thought was finally brought about when in the nineteenth century both periosteal and endosteal callus formation were shown to occur. The anatomical reposition of the fractured components and the stable fixation through osteosynthesis led to a novelty of the twentieth century: callus-free healing of bone fractures. Based on key texts and their authors, the zigzag course taken by our understanding of bone fracture healing and callus formation is illustrated.
    Notes: Zusammenfassung Bot die Geschichte der Frakturbehandlung immer wieder Anlass zur Darstellung, so hat die historische Entwicklung der Lehre von der Frakturheilung und Kallusbildung bisher wenig Beachtung gefunden. Zweifelte man in der Antike an der Regenerationsfähigkeit des Knochens und erklärte Galen das Festwerden des Knochenbruchs durch die Bildung eines aus der Nahrung stammenden Safts (Succus ossificus), der als Leim die Bruchenden verbinde, ohne selbst Knochen zu werden, so entbrannte im 18. Jahrhundert eine heftige Auseinandersetzung um den Ort und das Organ der inzwischen unbestrittenen Knochenneubildung. Machte Duhamel du Monceau (1700–1781) aufgrund seiner Beobachtungen an Bäumen das Periost für die Kallusbildung verantwortlich, so schrieben Albrecht von Haller (1708–1777) und seine Schüler bzw. Anhänger diese Funktion dem Succus ossificus und dem Knochenmark zu. Das im 17. Jahrhundert einsetzende Studium der experimentellen Frakturheilung mit Hilfe der Krappfärbung zeitigte im 18. Jahrhundert erste Früchte und brachte Licht in den komplexen Vorgang der Kallusbildung. Die unvereinbar scheinenden Positionen konnten im 19. Jahrhundert durch den Nachweis der periostalen und endostalen Kallusbildung überwunden werden. Die anatomische Reposition der Bruchenden und ihre stabile Fixation mit Hilfe der Osteosynthese bescherte dem 20. Jahrhundert das Novum einer kallusfreien Knochenbruchheilung. Anhand der Schlüsseltexte und ihrer Verfasser soll die keineswegs geradlinig verlaufende Entwicklung unserer Vorstellungen von der Knochenbruchheilung und Kallusbildung verständlich gemacht werden.

    Description / Table of Contents: Abstract Since antiquity low back pain spreading down to the buttock and the leg with or without neurological symptoms has repeatedly been described by medical authorities. To extract well defined nosological entities out of the medley of “sciatica” was a demanding process within a labyrinth of often misleading clues. Although the herniated intervertebral disk was recognised about the middle of the 19th century the causal nexus between intervertebral disk displacement and sciatica was only demonstrated in the thirties of the 20th century. The critical appraisal of pathomorphological lesions in patients suffering from sciatica was, as it still is, a challenging task even for experienced physicians. Key texts and their authors recall the main steps towards our modern concept of lumbar intervertebral disk displacement to the readers mind.
    Notes: Zusammenfassung Vom Rücken ins Gesäß und Bein ausstrahlende Schmerzen mit und ohne neurologische Ausfallserscheinungen wurden seit der Antike immer wieder von ärztlichen Autoritäten beschrieben. Die Abgrenzung pathogenetisch klar fassbarer Krankheitsbilder aus dem großen Sammeltopf der unspezifischen Hüft-Beinschmerzen (“Ischias”) war ein langwieriger, von Um- und Irrwegen gekennzeichneter Prozess. Trotz pathoanatomischer Kenntnis des Bandscheibenvorfalls um die Mitte des 19. Jahrhunderts wurde der Kausalzusammenhang zwischen lumbalem Bandscheibenvorfall und radikulären Symptomen erst in den 30er Jahren des 20. Jahrhunderts nachgewiesen. Aber nicht nur damals, auch heute noch bereitet die pathogenetische Wertung von pathomorphologischen Befunden beim “Ischias” selbst dem erfahrenen Orthopäden zuweilen Schwierigkeiten. Anhand ausgewählter Schlüsseltexte und ihrer Autoren werden wesentliche Schritte auf dem Weg zur Klärung des Krankheitsbildes des lumbalen Bandscheibenvorfalls in Erinnerung gerufen.

    Description / Table of Contents: Abstract Goal: Intracranial pressure (ICP) monitoring has a key role within the neuromonitoring, although ICP does not monitor processes of the central neuron directly and only with delay. One of the important factors in ICP monitoring is measurement accuracy. For a better understanding of ICP probes and their differences, the function and principles of intracranial pressure transducers should be evaluated from a technical point of view. Method: The principles of ICP measurement were analyzed and compared. Practical applications of these principles were examined and examples of different ICP probes were discussed regarding their mode of pressure transformation. The technical advances of ICP monitoring were analyzed. Results: Since LUNDBERG, a variety of different types of transducers has been developed. Ventricular ICP monitoring has been supplemented by extradural and intraparenchymatous probes. An increasing miniaturization of the transducers has emerged. Additionally, fiberoptic systems have been developed. Latest developments include multifunctional ICP probes. So far, the main problem of most types of transducers consists in the inability to assess measurement accuracy of a probe during the period of patient monitoring. Conclusion: ICP probes should be tested better for correct function by the manufacturer prior to sale. External controls of the measurement accuracy should be performed more frequently to ensure constant quality. Future ICP transducers have still to be more cost- effective.
    Notes: Zusammenfassung Fragestellung: Die Messung des intrakraniellen Drucks hat eine Schlüsselstellung innerhalb des Neuromonitorings erhalten, obwohl durch den Hirndruck die eigentlichen metabolischen Prozesse am zentralen Neuron nicht direkt und auch nur zeitlich verzögert registriert werden können. Voraussetzung für eine zuverlässige Hirndruckmessung sind spezifische Eigenschaften von Hirndrucksonden, die im folgenden untersucht werden sollen. Diese sollen dabei aus messtechnischer Sicht analysiert und kategorisiert werden, um ein besseres Verständnis für deren unterschiedliche Funktionsweise und Messeigenschaften zu erhalten. Methodik: Es werden die verschiedenen Messprinzipien, die bislang zur Anwendung kamen, dargelegt. Es wird aufgezeigt, wie diese Prinzipien praktisch genutzt werden. Probleme der Messgenauigkeit werden aus messtechnischer und klinischer Sicht erörtert. Ergebnisse: Es wurden seit Lundberg eine Vielzahl verschiedener Transducertypen entwickelt. Die ventrikuläre Hirndruckmessung wurde ergänzt durch epidurale und intraparenchymatöse Sonden. Eine zunehmende Miniaturisierung der Transducer und eine Verbesserung der Messtechnik hat eingesetzt. Zusätzlich wurden fiberoptische Systeme entwickelt. Neueste Entwicklungen zielen auf Multifunktionssonden ab, die zusätzlich zum intrakraniellen Druck auch die Hirntemperatur und weitere Parameter gleichzeitig messen können. Hauptproblem vieler der bisherigen Sonden ist das Fehlen einer direkten Kontrolle des Messverhaltens während des klinischen Einsatzes. Schlussfolgerungen: Sonden sollten vor Gebrauch vom Hersteller noch besser auf Funktionsfähigkeit überprüft und entsprechend ausgewiesen werden. Externe Kontrollen zur Überprüfung des Messverhaltens von intrakraniellen Drucksonden sollten zusätzlich verstärkt durchgeführt werden, um die Qualitätssicherung zu verbessern. Zukünftige intrakranielle Drucksonden müssen noch kostengünstiger werden.

    Description / Table of Contents: Georg Friedrich Stromeyer (1804– 1876) is generally known as one of the founders of orthopedics and orthopedic surgery and also made many contributions to modern military medicine. Furthermore, every oral and maxillofacial surgeon in Germany knows him because of the “Stromeyer hook”, which is used for elevation of zygomatic arch fractures. This special aspect as well as Stromeyer’s biography is presented in this article from the history of medicine.
    Notes: Georg Friedrich Stromeyer (1804–1876) wird nicht nur als Pionier der orthopädischen Chirurgie, sondern auch als ein Begründer der modernen Militärchirurgie angesehen. Jedem Mund-, Kiefer- und Gesichtschirurgen ist sein Name darüber hinaus durch die Verwendung des „Stromeyer-Hakens“ bei der Anhebung von Impressionsfrakturen des Jochbogens bekannt. Dieser Aspekt der Mund-, Kiefer- und Gesichtschirurgie sowie der Lebenslauf Stromeyers werden vorgestellt.

    Description / Table of Contents: Zusammenfassung Die Fortschritte in der Orthopädie in den letzten 10 Jahren sind beeindruckend. Weiterentwickelte diagnostische Methoden, wie Computertomographie, Szintigraphie und Arthroskopie haben dazu beigetragen. Die Mikrochirurgie eröffnete neue Dimensionen bei Extremitäten-erhaltenden Eingriffen. Neuentwicklungen beim künstlichen Gelenkersatz zeigen erfolgversprechende Resultate mit hoher Zuverlässigkeit. Die Anwendung neuer Operationstechniken setzt ein großes Detailwissen voraus und gehört deshalb in die Hand des Spezialisten.
    Notes: Summary The progress in orthopedic surgery during the last 10 years is very impressive. Further developed diagnostic methods such as computerized tomography, scintigraphy and arthroscopy have contributed to it. Microsurgery opened new dimensions in limb salvaging procedures. New developments of joint prostheses show promising results of a high reliability. The application of new operative techniques demands an extraordinary knowledge of details and therefore, should only be performed by a specialist.
