    Notes: Case history, skin tests and RAST were compared in a group (n= 40) of children with a history suggesting allergy to horses and in a group (n= 43) in whom there was no suspicion of hypersensitivity to horses.There was an agreement of 91% between case history and prick test. The same magnitude of agreement was found between case history and RAST (89%), and the agreement between RAST and prick test was 90%.The results of this investigation are clearly in contrast to earlier reports, in that there was a very good correlation between prick test, RAST and case history.The results suggest that: (a) a proper evaluation of the skin test is of critical importance for the degree of correlation with other diagnostic tests; and (b) RAST is as reliable as carefully performed and evaluated skin tests.

    Notes: Six patients with extrinsic allergic alveolitis had weak immediate and strong late reactions to intracutaneous injection of bird serum. Biopsy of the late reaction revealed vasculitis and immunoglobulin and complement deposition in one. The antibody responsible for the immediate hypersensitivity could be transferred in serum. It was short lasting (4 hr) and resistant to heat and 2-mercaptoethanol. No evidence of an immediate reaction in the lungs could be detected following inhalation challenge as judged by examination of spirometry, flow volume loops and single breath nitrogen washout curves.

    Notes: A double-blind, group comparative trial of 2% aqueous solution of sodium cromoglycate in seasonal allergic rhinitis (hay fever) is reported. All twenty-five patients in the active group found the treatment a success whereas fourteen out of nineteen in the placebo group regarded this as a failure. The method of application (metered dose spray) was acceptable to all patients.

    Notes: This study was undertaken to determine the proper use of skin tests with food extracts in diagnosis of hypersensitivity to food in children. Cutaneous reactions evoked by graded amounts of food extracts were compared with results of double-blind food challenge and in vitro release of histamine from leucocytes. A 3 mm or greater weal reaction in skin tests by puncture technique using food extracts of 1:20 w/v concentration was found to indicate the degree of hypersensitivity likely to be associated with clinically significant hypersensitivity reactions to food. Proper use of this simple technique will facilitate accurate diagnosis of food hypersensitivity in children by identifying the group among whom all positive reactions to food challenges will be found. Nevertheless, double-blind food challenge is essential to establish a diagnosis of symptomatic hypersensitivity to food.

    Notes: The grass pollen content of the air at four sites bordering the Bristol Channel (at Bristol, Cardiff, Penarth and Penmaen) was studied. The trap at Penmaen was situated at an official climatological station which provided the meteorological data used in the surveys. Although at certain times large differences could be seen between the concentrations of grass pollen at the different sites, overall the findings for the four sites were similar. Changes in the atmospheric pollen concentration related to variations in meteorological conditions. At the peak of the season high concentrations of grass pollen persisted throughout the night.

    Notes: Using the micropore filter technique of Boyden, the eosinophil chemotactic responses to an ECF-A tetrapeptide (Val-Gly-Ser-Glu) and histamine were studied in thirty-two patients with a peripheral blood eosinophilia. The eosinophilia was associated with extrinsic asthma (twelve patients), neoplasia (seven), helminth disease (five) and a miscellaneous group (eight) which included three patients with pulmonary eosinophilia, one of whom also had asthma. Both the tetrapeptide and histamine gave two types of dose–response. With Val-Gly-Ser-Glu this was either a single peak at 10-8, or two peaks at 10-4 and 10-7 mol/1, respectively. Histamine gave either a linear dose–response with maximal chemotaxis at the highest concentration or a peak response at 10-5 mol/1 with inhibition at higher doses. The two types of response given by the peptide were not related to the disease state and were reproducible when tested on more than one occasion. However, with histamine, linear dose–responses were observed in ten out of twelve patients with asthma, four out of five with helminth disease and two of the three with pulmonary eosinophilia. This was also reproducible when tested on subsequent occasions. Therefore, in these diseases, which are known to be associated with exogenous antigens and raised IgE levels, eosinophils from 80% of the patients studied (sixteen out of twenty) gave a linear rather than a ‘bell-shaped’ response to histamine. In contrast, only four of twelve patients(33%) with eosinophilia in association with other diseases (which included seven with neoplasia) gave this type of response. If this observation can be confirmed with larger numbers of patients it may be useful diagnostically in eosinophilia of unknown origin.

    Notes: A method for determining the amount of total IgE bound to leucocytes has been devised by measuring that released on incubating various cell number dilutions in acetate buffer pH 4.0. A linear relationship between IgE released and cell number was obtained over a limited concentration range. The method seemed to work satisfactorily with 50 ml of whole blood for those subjects with very high serum levels of total IgE. However, with subjects having a slightly elevated or normal serum IgE level it may be necessary to use twice the amount of blood.

    Notes: A comparison was made between the effects of sodium cromoglycate (SCG) and placebo on symptoms of food allergy in twenty children. Both the clinician's and patient's preferences and the clinician's evaluation of the specific response to challenge showed a significant benefit from SCG. A striking effect on skin symptoms was seen in some children and the recorded mean score for skin symptoms was lower after 2 weeks’ treatment with sodium cromoglycate. However, the differences detected from the mean symptom scores were not statistically significant. Neither were there any significant changes in the IgE, IgG, IgA and IgM food allergen antibody levels related to the treatment.

    Notes: A method is presented for measuring the uptake of Aspergillus fumigatus protein by IgG antibodies from human serum. The human IgG is first isolated with an anti-IgG conjugated immunosorbent and then incubated with radio-iodinated A. fumigatus protein. A group of twenty-three control sera gave the same background levels as tests without sera. The highest uptake of A. fumigatus protein was given by the sera of patients with aspergilloma, and lower values were obtained with six out of thirteen sera from patients with asthma and twenty-five out of twenty-eight sera from patients with asthma and pulmonary eosinophilia, i.e. allergic bronchopulmonary aspergillosis. These results were in agreement with precipitin tests. Inhibition studies with unrelated antigens showed that the reactions were specific for the A. fumigatus protein, of which more was bound by test sera than with a crude extract.
